dbefa1f31a Rename .data.once to .data..once to fix resetting WARN*_ONCE
Commit b1fca27d38 ("kernel debug: support resetting WARN*_ONCE")
added support for clearing the state of once warnings. However,
it is not functional when CONFIG_LD_DEAD_CODE_DATA_ELIMINATION or
CONFIG_LTO_CLANG is enabled, because .data.once matches the
.data.[0-9a-zA-Z_]* pattern in the DATA_MAIN macro.

Commit cb87481ee8 ("kbuild: linker script do not match C names unless
LD_DEAD_CODE_DATA_ELIMINATION is configured") was introduced to suppress
the issue for the default CONFIG_LD_DEAD_CODE_DATA_ELIMINATION=n case,
providing a minimal fix for stable backporting. We were aware this did
not address the issue for CONFIG_LD_DEAD_CODE_DATA_ELIMINATION=y. The
plan was to apply correct fixes and then revert cb87481ee8. [1]

Seven years have passed since then, yet the #ifdef workaround remains in
place. Meanwhile, commit b1fca27d38 introduced the .data.once section,
and commit dc5723b02e ("kbuild: add support for Clang LTO") extended
the #ifdef.

Using a ".." separator in the section name fixes the issue for
CONFIG_LD_DEAD_CODE_DATA_ELIMINATION and CONFIG_LTO_CLANG.

b50e195ff4 mm: update validate_mm() to use vma iterator
Use the vma iterator in the validation code and combine the code to check
the maple tree into the main validate_mm() function.

Introduce a new function vma_iter_dump_tree() to dump the maple tree in
hex layout.

Replace all calls to validate_mm_mt() with validate_mm().

a738e9bad6 mm/debug: Provide VM_WARN_ON_IRQS_ENABLED()
Some places in the VM code expect interrupts disabled, which is a valid
expectation on non-PREEMPT_RT kernels, but does not hold on RT kernels in
some places because the RT spinlock substitution does not disable
interrupts.

To avoid sprinkling CONFIG_PREEMPT_RT conditionals into those places,
provide VM_WARN_ON_IRQS_ENABLED() which is only enabled when VM_DEBUG=y and
PREEMPT_RT=n.

020bc44a9f ceph: switch back to testing for NULL folio->private in ceph_dirty_folio
Willy requested that we change this back to warning on folio->private
being non-NULl. He's trying to kill off the PG_private flag, and so we'd
like to catch where it's non-NULL.

Add a VM_WARN_ON_FOLIO (since it doesn't exist yet) and change over to
using that instead of VM_BUG_ON_FOLIO along with testing the ->private
pointer.

d2f07ec052 mm: make __dump_page static
Patch series "Constify struct page arguments".

While working on various solutions to the 32-bit struct page size
regression, one of the problems I found was the networking stack expects
to be able to pass const struct page pointers around, and the mm doesn't
provide a lot of const-friendly functions to call.  The root tangle of
problems is that a lot of functions call VM_BUG_ON_PAGE(), which calls
dump_page(), which calls a lot of functions which don't take a const
struct page (but could be const).

This patch (of 6):

The only caller of __dump_page() now opencodes dump_page(), so remove it
as an externally visible symbol.

a405588862 mm/memcg: warning on !memcg after readahead page charged
Add VM_WARN_ON_ONCE_PAGE() macro.

Since readahead page is charged on memcg too, in theory we don't have to
check this exception now.  Before safely remove them all, add a warning
for the unexpected !memcg.

91241681c6 include/linux/mmdebug.h: make VM_WARN* non-rvals
At present the construct

	if (VM_WARN(...))

will compile OK with CONFIG_DEBUG_VM=y and will fail with
CONFIG_DEBUG_VM=n.  The reason is that VM_{WARN,BUG}* have always been
special wrt.  {WARN/BUG}* and never generate any code when DEBUG_VM is
disabled.  So we cannot really use it in conditionals.

We considered changing things so that this construct works in both cases
but that might cause unwanted code generation with CONFIG_DEBUG_VM=n.
It is safer and simpler to make the build fail in both cases.

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   SPDX license identifier                            # files

   and resulted in the first patch in this series.

   If that file was a */uapi/* path one, it was "GPL-2.0 WITH
   Linux-syscall-note" otherwise it was "GPL-2.0".  Results of that was:

 - if a file had some form of licensing information in it, and was one

   and that resulted in the third patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

ff8e811638 mm, debug: move bad flags printing to bad_page()
Since bad_page() is the only user of the badflags parameter of
dump_page_badflags(), we can move the code to bad_page() and simplify a
bit.

The dump_page_badflags() function is renamed to __dump_page() and can
still be called separately from dump_page() for temporary debug prints
where page_owner info is not desired.

The only user-visible change is that page->mem_cgroup is printed before
the bad flags.

95ad97554a page-flags: introduce page flags policies wrt compound pages
This patch adds a third argument to macros which create function
definitions for page flags.  This argument defines how page-flags
helpers behave on compound functions.

For now we define four policies:

 - PF_ANY: the helper function operates on the page it gets, regardless
   if it's non-compound, head or tail.

 - PF_HEAD: the helper function operates on the head page of the
   compound page if it gets tail page.

 - PF_NO_TAIL: only head and non-compond pages are acceptable for this
   helper function.

 - PF_NO_COMPOUND: only non-compound pages are acceptable for this
   helper function.

For now we use policy PF_ANY for all helpers, which matches current
behaviour.

We do not enforce the policy for TESTPAGEFLAG, because we have flags
checked for random pages all over the kernel.  Noticeable exception to
this is PageTransHuge() which triggers VM_BUG_ON() for tail page.

e4f674229c mm: pass VM_BUG_ON() reason to dump_page()
I recently added a patch to let folks pass a "reason" string dump_page()
which gets dumped out along with the page's data.  This essentially
saves the bug-reader a trip in to the source to figure out why we
BUG_ON()'d.

The new VM_BUG_ON_PAGE() passes in NULL for "reason".  It seems like we
might as well pass the BUG_ON() condition if we have it.  This will
bloat kernels a bit with ~160 new strings, but this is all under a
debugging option anyway.

I tried to use 'dump_page(page, __func__)' for debugging, but it triggers
warning:

  warning: passing argument 2 of `dump_page' discards `const' qualifier from pointer target type [enabled by default]

Let's convert 'reason' to 'const char *' in dump_page() and friends: we
shouldn't modify it anyway.

309381feae mm: dump page when hitting a VM_BUG_ON using VM_BUG_ON_PAGE
Most of the VM_BUG_ON assertions are performed on a page.  Usually, when
one of these assertions fails we'll get a BUG_ON with a call stack and
the registers.

I've recently noticed based on the requests to add a small piece of code
that dumps the page to various VM_BUG_ON sites that the page dump is
quite useful to people debugging issues in mm.

This patch adds a VM_BUG_ON_PAGE(cond, page) which beyond doing what
VM_BUG_ON() does, also dumps the page before executing the actual
BUG_ON.

02602a18c3 bug: completely remove code generated by disabled VM_BUG_ON()
Even if CONFIG_DEBUG_VM=n gcc genereates code for some VM_BUG_ON()

for example VM_BUG_ON(!PageCompound(page) || !PageHead(page)); in
do_huge_pmd_wp_page() generates 114 bytes of code.

But they mostly disappears when I split this VM_BUG_ON into two:

  -VM_BUG_ON(!PageCompound(page) || !PageHead(page));
  +VM_BUG_ON(!PageCompound(page));
  +VM_BUG_ON(!PageHead(page));

weird... but anyway after this patch code disappears completely.

Add some (configurable) expensive sanity checking to catch wrong address
translations on x86.

- create linux/mmdebug.h file to be able include this file in
  asm headers to not get unsolvable loops in header files
- __phys_addr on x86_32 became a function in ioremap.c since
  PAGE_OFFSET, is_vmalloc_addr and VMALLOC_* non-constasts are undefined
  if declared in page_32.h
- add __phys_addr_const for initializing doublefault_tss.__cr3

Tested on 386, 386pae, x86_64 and x86_64 numa=fake=2.

Contains Andi's enable numa virtual address debug patch.
