- Do not create PT_LOAD program header for the kenel image when
   the virtual memory informaton in OS_INFO data is not available.
   That fixes stand-alone dump failures against kernels that do not
   provide the virtual memory informaton

Pull s390 fixes from Alexander Gordeev:
