Copy directly from the se_cmd CDB to the one in the scsi_request. This
temporarily limits the pscsi backend to supporting only up to 16 byte CDBs,
but this restriction will be lifted later in this series.

Make cmd_ring_size changeable similar to the way it is done for
max_data_area_mb. The reason is that our tcmu client will create thousands
of tcmu instances, and this will consume lots of mem with default 8Mb cmd
ring size for every backstore.
One can change the value by typing:
echo "cmd_ring_size_mb=N" > control
The "N" is a integer between 1 to 8, if set 1, the cmd ring can hold about
6k cmds(tcmu_cmd_entry about 176 byte) at least.
The value is printed when doing:
cat info
In addition, a new readonly attribute 'cmd_ring_size_mb' returns the value
in read.

bio_alloc to optimize the assignment. NULL/0 can be passed, both for the
passthrough case on a raw request_queue and to temporarily avoid
refactoring some nasty code.
Also move the gfp_mask argument after the nr_vecs argument for a much
more logical calling convention matching what most of the kernel does.

iscsit_tpg_check_network_portal() has nested for_each loops and is supposed
to return true when a match is found. However, the tpg loop will still
continue after existing the tpg_np loop. If this tpg_np is not the last the
match value will be changed.
Break the outer loop after finding a match and make sure the np under each
tpg is unique.

The "item" pointer is always going to be valid pointer and does not need to
be checked.
But if "item" were NULL then item_to_lun() would not return a NULL, but
instead, the container_of() pointer math would return a value in the error
pointer range. This confuses static checkers since it looks like a NULL vs
IS_ERR() bug.
Delete the bogus checks.

Currently TMF commands are removed from de_device.dev_tmf_list at the very
end of se_cmd lifecycle. However, se_lun unlinks from se_cmd upon a command
status (response) being queued in transport layer. This means that LUN and
backend device can be deleted in the meantime and a panic will occur:

For the iSCSI protocol this is easily reproduced:
- Send some SCSI sommand
- Send Abort of that command over iSCSI
- Remove LUN on target
- Send next iSCSI command to acknowledge the Abort_Response
- Target panics
There is no need to keep the command in tmr_list until response completion,
so move the removal from tmr_list from the response completion to the
response queueing when the LUN is unlinked. Move the removal from state
list too as it is a subject to the same race condition.

The second argument was only used by the USB gadget code, yet everyone
pays the overhead of passing a zero to be passed into aio, where it
ends up being part of the aio res2 value.

Add a new helper that calls blk_get_request and initializes the
scsi_request to avoid the indirect call through ->.initialize_rq_fn.
Note that this makes the pktcdvd driver depend on the SCSI core, but
given that only SCSI devices support SCSI passthrough requests that
is not a functional change.

When userspace changes the LUN's ALUA group, it will set the LUN's group to
NULL then to the new group. Before the new group is set,
target_alua_state_check() will return 0 and allow the I/O to execute. This
has us skip the NULL stage, and just swap in the new group.

We are only holding the lun_tg_pt_gp_lock in target_alua_state_check() to
make sure tg_pt_gp is not freed from under us while we copy the state,
delay, ID values. We can instead use RCU here to access the tg_pt_gp.
With this patch IOPs can increase up to 10% for jobs like:

--ioengine=libaio --iodepth=64 --numjobs=N
when there are multiple sessions (running that fio command to each /dev/sdX
or using multipath and there are over 8 paths), or more than 8 queues for

We can't free the tg_pt_gp in core_alua_set_tg_pt_gp_id() because it's
still accessed via configfs. Its release must go through the normal
configfs/refcount process.
The max alua_tg_pt_gps_count check should probably have been done in
core_alua_allocate_tg_pt_gp(), but with the current code userspace could
have created 0x0000ffff + 1 groups, but only set the id for 0x0000ffff.
Then it could have deleted a group with an ID set, and then set the ID for
that extra group and it would work ok.
It's unlikely, but just in case this patch continues to allow that type of
behavior, and just fixes the kfree() while in use bug.

This patch fixes the following bugs:
1. If there are multiple ordered cmds queued and multiple simple cmds
completing, target_restart_delayed_cmds() could be called on different
CPUs and each instance could start a ordered cmd. They could then run in
different orders than they were queued.
2. target_restart_delayed_cmds() and target_handle_task_attr() can race
where:
1. target_handle_task_attr() has passed the simple_cmds == 0 check.
2. transport_complete_task_attr() then decrements simple_cmds to 0.
3. transport_complete_task_attr() runs target_restart_delayed_cmds() and
it does not see any cmds on the delayed_cmd_list.
4. target_handle_task_attr() adds the cmd to the delayed_cmd_list.
The cmd will then end up timing out.
3. If we are sent > 1 ordered cmds and simple_cmds == 0, we can execute
them out of order, because target_handle_task_attr() will hit that
simple_cmds check first and return false for all ordered cmds sent.
4. We run target_restart_delayed_cmds() after every cmd completion, so if
there is more than 1 simple cmd running, we start executing ordered cmds
after that first cmd instead of waiting for all of them to complete.
5. Ordered cmds are not supposed to start until HEAD OF QUEUE and all older
cmds have completed, and not just simple.
6. It's not a bug but it doesn't make sense to take the delayed_cmd_lock
for every cmd completion when ordered cmds are almost never used. Just
replacing that lock with an atomic increases IOPs by up to 10% when
completions are spread over multiple CPUs and there are multiple
sessions/ mqs/thread accessing the same device.
This patch moves the queued delayed handling to a per device work to
serialze the cmd executions for each device and adds a new counter to track
HEAD_OF_QUEUE and SIMPLE cmds. We can then check the new counter to
determine when to run the work on the completion path.

We can race where target_handle_task_attr() has put the cmd on the
delayed_cmd_list. Then target_restart_delayed_cmds() has removed it and set
CMD_T_SENT, but then target_execute_cmd() now clears that bit.
This patch moves the clearing to before we've put the cmd on the list.

Tcmu populates the data area (used for communication with userspace) with
pages that are allocated by calling alloc_page(GFP_NOIO). Therefore
previous content of the allocated pages is exposed to user space. Avoid
this by adding __GFP_ZERO flag.
Zeroing the pages does (nearly) not affect tcmu throughput, because
allocated pages are re-used for the data transfers of later SCSI cmds.

Provide a way to remove code duplication in the fabric modules and

Change the log level of the following message to debug:
Unsupported SCSI Opcode 0xXX, sending CHECK_CONDITION.
This message is mostly helpful during debugging sessions in order to
understand errors on the initiator side. But most of the time it's just
useless and makes reading logs much harder.
It gets particularly annoying if there are many initiators that come and go
or if an initiator runs a program that does not care whether the command is
supported and just keeps sending it.

Commit 356ba2a8bc ("scsi: target: tcmu: Make pgr_support and alua_support
attributes writable") introduced support for changeable alua_support and
pgr_support target attributes. These can only be changed if the backstore
is user-backed, otherwise the kernel returns -EINVAL.
This triggers a warning in the targetcli/rtslib code when performing a
target restore that includes non-userbacked backstores:

Fix this warning by returning an error code only if we are really going to

Return ENOSYS instead of EINVAL if the pgr/alua attributes can not be
changed, this way it will be possible for userspace to understand if the
operation failed because an invalid value has been passed to strtobool() or
because the attributes are fixed.

churn was replacing the command request field request with a macro,
allowing us to offset map to it and remove the redundant field; the
same was also done for the tag field. The most impactful change is

decade.

Currently, backend drivers can fail I/O with SAM_STAT_CHECK_CONDITION which
gets us TCM_LOGICAL_UNIT_COMMUNICATION_FAILURE.
Add a new helper that allows backend drivers to fail with specific sense
codes.

The return value of transport_kmap_data_sg() is assigned to the variable
buf:
buf = transport_kmap_data_sg(cmd);
And then it is checked:

This indicates that buf can be NULL. However, it is dereferenced in the
following statements:

To fix these possible null-pointer dereferences, dereference buf and call

When running command pipelining for WRITE direction commands (e.g. tape
device write), userspace sends cmd completion to cmd ring before processing
write data. In that case userspace has to copy data before sending
completion, because cmd completion also implicitly releases the data buffer
in data area.
The new feature KEEP_BUF allows userspace to optionally keep the buffer
after completion by setting new bit TCMU_UFLAG_KEEP_BUF in
tcmu_cmd_entry_hdr->uflags. In that case buffer has to be released
explicitly by writing the cmd_id to new action item free_kept_buf.
All kept buffers are released during reset_ring and if userspace closes uio
device (tcmu_release).

The driver core ignores the return value of this callback because there
is only little it can do when a device disappears.
This is the final bit of a long lasting cleanup quest where several
buses were converted to also return void from their remove callback.
Additionally some resource leaks were fixed that were caused by drivers
returning an error code in the expectation that the driver won't go
away.
With struct bus_type::remove returning void it's prevented that newly
implemented buses return an ignored error code and so don't anticipate
wrong expectations for driver authors.

CPU affinity control added with commit 39ae3edda3 ("scsi: target: core:
Make completion affinity configurable") makes target_complete_cmd() queue
work on a CPU based on se_tpg->se_tpg_wwn->cmd_compl_affinity state.
LIO's EXTENDED COPY worker is a special case in that read/write cmds are
dispatched using the global xcopy_pt_tpg, which carries a NULL se_tpg_wwn
pointer following initialization in target_xcopy_setup_pt().
The NULL xcopy_pt_tpg->se_tpg_wwn pointer is dereferenced on completion of
any EXTENDED COPY initiated read/write cmds. E.g using the libiscsi

On realtime kernels, spin_lock_irq*(spinlock_t) do not disable the
interrupts, a call to irqs_disabled() will return false thus firing a
warning in __transport_wait_for_tasks().
Remove the warning and also replace assert_spin_locked() with
lockdep_assert_held()

Replace the check for DRIVER_SENSE with a check for
scsi_status_is_check_condition().
Audit all callsites to ensure the SAM status is set correctly. For
backwards compability move the DRIVER_SENSE definition to sg.h, and update
sg, bsg, and scsi_ioctl to set the DRIVER_SENSE driver_status whenever
SAM_STAT_CHECK_CONDITION is present.

This has us use raw_smp_processor_id() in iblock's plug_device callout.
smp_processor_id() is not needed here, because we are running from a per
CPU work item that is also queued to run on a worker thread that is
normally bound to a specific CPU. If the worker thread did end up switching
CPUs then it's handled the same way we handle when the work got moved to a
different CPU's worker thread, where we will just end up sending I/O from
the new CPU.

Extend the standard INQUIRY data to 96 bytes and fill in the VERSION
DESCRIPTOR fields.
The layout follows SPC-4:
- SCSI architecture standard
- SCSI transport protocol standard
- SCSI primary command set standard
- SCSI device type command set standard
All version descriptor values are defined as "no version claimed" because
some initiators fail to recognize anything else.

The structure pointer passed to container_of() is never NULL; that was
already checked. That means that the result of container_of() operations
on it is also never NULL, even though se_node_acl is the first element
of the structure embedding it. On top of that, it is misleading to perform
a NULL check on the result of container_of() because the position of the
contained element could change, which would make the test invalid.
Remove the unnecessary NULL check.
As it turns out, the container_of operation was only made for the purpose
of the NULL check. If the container_of is actually needed, it is repeated
later. Remove the container_of operation as well.
The NULL check was identified and removed with the following Coccinelle

If tcmu_handle_completions() finds an invalid cmd_id while looping over cmd
responses from userspace it sets TCMU_DEV_BIT_BROKEN and breaks the
loop. This means that it does further handling for the tcmu device.
Skip that handling by replacing 'break' with 'return'.
Additionally change tcmu_handle_completions() from unsigned int to bool,
since the value used in return already is bool.

Replace DATA_PAGES_PER_BLK and DATA_BLOCK_SIZE with new struct elements
tcmu_dev->data_pages_per_blk and tcmu_dev->data_blk_size. These new
variables are still loaded with constant definition DATA_PAGES_PER_BLK_DEF
(= 1) and DATA_PAGES_PER_BLK_DEF * PAGE_SIZE.
There is no way yet to set the values via configfs.

Change tcmu to support DATA_BLOCK_SIZE being a multiple of PAGE_SIZE. There
are two reasons why one would like to have a bigger DATA_BLOCK_SIZE:
1) If userspace - e.g. due to data compression, encryption or
deduplication - needs to have receive or transmit data in a consecutive
buffer, we can define DATA_BLOCK_SIZE to the maximum size of a SCSI
READ/WRITE to enforce that userspace sees just one consecutive
buffer. That way we can avoid the need for doing data copy in
userspace.
2) Using a bigger data block size can speed up command processing in
tcmu. The number of free data blocks to look up in bitmap is reduced
substantially. The lookup for data pages in radix_tree can be done more
efficiently if there are multiple pages in a data block. The maximum
number of IOVs to set up is lower so cmd entries in the ring become
smaller.

Rename some variables and definitions as a first preparation for
DATA_BLOCK_SIZE != PAGE_SIZE and add the new DATA_PAGES_PER_BLK definition
containing the number of pages per data block.
Rename tcmu_try_get_block_page() to tcmu_try_get_data_page(). Keep name
tcmu_get_block_page() since it will go away in a following commit when
there is only one caller left. Subsequent commits will then add full
support for DATA_PAGES_PER_BLK != 1, which also means DATA_BLOCK_SIZE =
DATA_PAGES_PER_BLK * PAGE_SIZE

Some definitions and members of struct tcmu_dev had misleading
names. Examples:
- ring_size was used for the size of mailbox + cmd ring + data area
- CMDR_SIZE was used for size of mailbox + cmd ring
I added the new definition MB_CMDR_SIZE (mailbox + command ring), changed
CMDR_SIZE to hold the size of the command ring only and replaced in struct
tcmu_dev the member ring_size with mmap_pages, because the member is now
used in tcmu_mmap() only, where we need page count, not size.
I also added the new struct tcmu_dev member 'cmdr' which is used to replace
some occurences of '(void *)mb + CMDR_OFF' with 'udev->cmdr' for better
readability.

This commit adds the DUMMY flag to the rd_mcp backend that forces a logical
unit to report itself as not connected device of an unknown type.
Essentially this allows users to create devices identical to the device for
the virtual LUN 0, making it possible to explicitly create a LUN 0 device
and configure its WWNs (e.g. vendor or product name).

pscsi_map_sg() uses the variable nr_pages as a hint for bio_kmalloc() how
many vector elements to allocate. If nr_pages is < BIO_MAX_PAGES, it will
be reset to 0 after successful allocation of the bio.
If bio_add_pc_page() fails later for whatever reason, pscsi_map_sg() tries
to allocate another bio, passing nr_vecs = 0. This causes bio_add_pc_page()
to fail immediately in the next call. pci_map_sg() continues to allocate
zero-length bios until memory is exhausted and the kernel crashes with
OOM. This can be easily observed by exporting a SATA DVD drive via pscsi.
The target crashes as soon as the client tries to access the DVD LUN. In
the case I analyzed, bio_add_pc_page() would fail because the DVD device's
max_sectors_kb (128) was exceeded.
Avoid this by simply not resetting nr_pages to 0 after allocating the
bio. This way, the client receives an I/O error when it tries to send
requests exceeding the devices max_sectors_kb, and eventually gets it
right. The client must still limit max_sectors_kb e.g. by an udev rule if
(like in my case) the driver doesn't report valid block limits, otherwise
it encounters I/O errors.

In commit f7c89771d0 ("scsi: target: tcmu: Replace radix_tree with
XArray") the meaning of last parameter of tcmu_blocks_release() was
changed. So in the callers we should subtract 1 from the previous
parameter.
Unfortunately that change got lost at one of the two places where
tcmu_blocks_release() is called. That does not lead to any problems, but we
should adjust it anyway.

The two arguments to the functions op and opf which are REQ_OP_XXX and

Especially when using tcmu with tcm_loop, memory allocations with
GFP_KERNEL for a LUN can cause write back to the same LUN.
So we have to use GFP_NOIO when allocation is done while handling commands
or while holding cmdr_lock.

It may not always be best to complete the IO on same CPU as it was
submitted on. This commit allows userspace to configure it.
This has been useful for vhost-scsi where we have a single thread for
submissions and completions. If we force the completion on the submission
CPU we may be adding conflicts with what the user has setup in the lower
levels with settings like the block layer rq_affinity or the driver's IRQ
or softirq (the network's rps_cpus value) settings.
We may also want to set it up where the vhost thread runs on CPU N and does
its submissions/completions there, and then have LIO do its completion
booking on CPU M, but can't configure the lower levels due to issues like
using dm-multipath with lots of paths (the path selector can throw commands
all over the system because it's only taking into account latency/throughput
at its level).
The new setting is in:
/sys/kernel/config/target/$fabric/$target/param/cmd_completion_affinity
Writing:
-1 -> Gives the current default behavior of completing on the
submission CPU.
-2 -> Completes the cmd on the CPU the lower layers sent it to us from.
> 0 -> Completes on the CPU userspace has specified.

If a cmd is on the submission workqueue then the TMR code will miss it, and
end up returning task not found or success for LUN resets. The fabric
driver might then tell the initiator that the running cmds have been
handled when they are about to run.
This adds a flush when we are processing TMRs to make sure queued cmds do
not run after returning the TMR response.

This patch adds plug/unplug callouts for tcmu, so we can avoid the number
of times we switch to userspace. Using this driver with tcm_loop is a
common config, and dependng on the nr_hw_queues (nr_hw_queues=1 performs
much better) and fio jobs (lower num jobs around 4) this patch can increase
IOPS by only around 5-10% because we hit other issues like the big per tcmu
device mutex.

This patch adds plug/unplug callouts for iblock. For an initiator driver
like iSCSI which wants to pass multiple cmds to its xmit thread instead of
one cmd at a time, this increases IOPS by around 10% with vhost-scsi
(combined with the last patches we can see a total 40-50% increase). For
driver combos like tcm_loop and faster drivers like the iSER initiator, we
can still see IOPS increase by 20-30% when tcm_loop's nr_hw_queues setting
is also increased.

target_core_iblock is plugging and unplugging on every command and this is
causing perf issues for drivers that prefer batched cmds. With recent
patches we can now take multiple cmds from a fabric driver queue and then
pass them down the backend drivers in a batch. This patch adds this support
by adding 2 callouts to the backend for plugging and unplugging the
device. Subsequent commits will add support for iblock and tcmu device
plugging.

loop and vhost/scsi do their target cmd submission from driver
workqueues. This allows them to avoid an issue where the backend may block
waiting for resources like tags/requests, mem/locks, etc and that ends up
blocking their entire submission path and for the case of vhost-scsi both
the submission and completion path.
This patch adds a helper drivers can use to submit from a LIO workqueue.
This code will then be extended in the next patches to fix the plugging of
backend devices.
We are only converting vhost/loop initially, but the workqueue based
submission will work for other drivers and have similar benefits where the
main target loops will not end up blocking one some backend resource.

tcm_loop could be used like a normal block device, so we can't use
GFP_KERNEL and should use GFP_NOIO. This adds a gfp_t arg to
target_cmd_init_cdb() and converts the users. For every driver but loop
GFP_KERNEL is kept.
This will also be useful in subsequent patches where loop needs to do
target_submit_prep() from interrupt context to get a ref to the se_device,
and so it will need to use GFP_ATOMIC.

target_submit_cmd() is now only for simple drivers that do their own sync
during shutdown and do not use target_stop_session().
tcm_fc uses target_stop_session() to sync session shutdown with LIO core,
so we use target_init_cmd(), target_submit_prep(), target_submit(), because
target_init_cmd() will now detect the target_stop_session() call and return
an error.

This breaks up target_submit_cmd_map_sgls() into 3 helpers:
- target_init_cmd(): Do the basic general setup and get a refcount to the
session to make sure the caller can execute the cmd.
- target_submit_prep(): Do the mapping, cdb processing and get a ref to
the LUN.
- target_submit(): Pass the cmd to LIO core for execution.
The above functions must be used by drivers that either:
1. Rely on LIO for session shutdown synchronization by calling
target_stop_session().
2. Need to map sgls.
When the next patches are applied then simple drivers that do not need the
extra functionality above can use target_submit_cmd() and not worry about
failures being returned and how to handle them, since many drivers were
getting this wrong and would have hit refcount bugs.
Also, by breaking target_submit_cmd_map_sgls() up into these 3 helper
functions, we can allow the later patches to do the init/prep from
interrupt context and then do the submission from a workqueue.

The kref_get_unless_zero() use in target_get_sess_cmd() was added in:
commit 1b4c59b7a1 ("target: fix potential race window in
target_sess_cmd_list_waiting()")'
but it does not seem to do anything.
The original patch might have thought we could have added the cmd to the
sess_wait_list and then target_wait_for_sess_cmds could do a put before
target_get_sess_cmd did its get. That wouldn't happen because we do the get
first then grab the sess lock and put it on the list.
It is also not needed now, because the sess_cmd_list does not exist anymore
and we instead wait on the session cmd_count.
The other problem with the commit is that several
target_submit_cmd_map_sgls()/target_submit_cmd() callers do not handle the
error case properly if it were to ever happen. These drivers think they
have their normal refcount on the cmd and in many cases do a
transport_generic_free_cmd() plus target_put_sess_cmd() so they would have
fired off the refcount WARN/BUGs.
This patch just changes the kref_get_unless_zero() to kref_get().

Prepare to split target_submit_cmd_map_sgls() so the initialization and
submission part can be called at different times. If the init part fails we
can reference the t_task_cdb early in some of the logging and tracing
code. Move it to transport_init_se_cmd() so we don't hit NULL pointer
crashes.

It's often inconvenient to use BIO_MAX_PAGES due to min() requiring the

When user deletes a tcmu device via configFS, tcmu calls
uio_unregister_device(). During that call uio resets its pointer to struct
uio_info provided by tcmu. That means, after uio_unregister_device() uio
will no longer execute any of the callbacks tcmu had set in uio_info.
Especially, if userspace daemon still holds the corresponding uio device
open or mmap'ed while tcmu calls uio_unregister_device(), uio will not call
tcmu_release() when userspace finally closes and munmaps the uio device.
Since tcmu does refcounting for the tcmu device in tcmu_open() and
tcmu_release(), in the decribed case refcount does not drop to 0 and tcmu
does not free tcmu device's resources. In extreme cases this can cause
memory leaking of up to 1 GB for a single tcmu device.
After uio_unregister_device(), uio will reject every open, read, write,
mmap from userspace with -EOI. But userspace daemon can still access the
mmap'ed command ring and data area. Therefore tcmu should wait until
userspace munmaps the uio device before it frees the resources, as we don't
want to cause SIGSEGV or SIGBUS to user space.
That said, current refcounting during tcmu_open and tcmu_release does not
work correctly, and refcounting better should be done in the open and close
callouts of the vm_operations_struct, which tcmu assigns to each mmap of
the uio device (because it wants its own page fault handler).
This patch fixes the memory leak by removing refcounting from tcmu_open and
tcmu_close, and instead adding new tcmu_vma_open() and tcmu_vma_close()
handlers that only do refcounting.

TCM buffer length doesn't necessarily equal 8 + ADDITIONAL LENGTH which
might be considered an underflow in case of Data-In size being greater than
8 + ADDITIONAL LENGTH. So truncate buffer length to prevent underflow.

TCM doesn't properly handle underflow case for service actions. One way to
prevent it is to always complete command with
target_complete_cmd_with_length(), however it requires access to data_sg,
which is not always available.
This change introduces target_set_cmd_data_length() function which allows
to set command data length before completing it.

The Max imm data size in cxgb4 is not similar to the max imm data size
in the chtls. This caused an mismatch in output of is_ofld_imm() of
cxgb4 and chtls. So fixed this by keeping the max wreq size of imm data
same in both chtls and cxgb4 as MAX_IMM_OFLD_TX_DATA_WR_LEN.
As cxgb4's max imm. data value for ofld packets is changed to
MAX_IMM_OFLD_TX_DATA_WR_LEN. Using the same in cxgbit also.

If the command requested that data beyond the length specified by the

According to RFC 7143 11.4.5.2.:
If SPDTL > EDTL for a task, iSCSI Overflow MUST be signaled in the SCSI
Response PDU as specified in Section 11.4.5.1. The Residual Count MUST
be set to the numerical value of (SPDTL - EDTL).
If SPDTL < EDTL for a task, iSCSI Underflow MUST be signaled in the SCSI
Response PDU as specified in Section 11.4.5.1. The Residual Count MUST
be set to the numerical value of (EDTL - SPDTL).
libiscsi has residual write tests that check residual kind and residual
amount and all of them (Write10Residuals, Write12Residuals,
Write16Residuals) currently fail.
One of the reasons why they fail is because target completes write commands
with INVALID FIELD IN CDB before setting the Overflow/Underflow bit and
residual amount.
Set the Overflow/Underflow bit and the residual amount before failing a
write to comply with RFC 7143.

TCM always fails SBC commands with residuals for 4Kn devices when the
command is processed by sbc_parse_cdb(). That prevents residual signalling
to the transport driver because residual kind and residual amount aren't
set. It also makes residual handling different from 512-byte formatted
devices - if there are residuals 512-byte LUN would proceed with command
execution while 4K-byte LUN would fail.

The UFS core has received a substantial rework this cycle. This in
turn has caused a merge conflict in linux-next. Merge 5.11/scsi-fixes
into 5.12/scsi-queue and resolve the conflict.

This saves one memory allocation, and ensures the bvecs aren't freed
before the AIO completion. This will allow the lower level code to be
optimized so that it can avoid allocating another bvec array.

The usage of in_interrupt() in drivers is phased out and Linus clearly
requested that code which changes behaviour depending on context should
either be separated or the context be conveyed in an argument passed by the
caller, which usually knows the context.
transport_handle_cdb_direct() has a comment saying that it may only be
invoked from process context. It invokes transport_generic_new_cmd() which
performs GFP_KERNEL memory allocations. in_interrupt() does not detect all
the contexts where it is invalid to sleep (for the blocking GFP_KERNEL
allocation) as it fails to detect sections with disabled preemption.
Replace the in_interrupt() based check with a might_sleep() annotation.

The return value of iscsit_check_session_usage_count() is only checked if
it was not allowed to sleep. If it returns `2' then a timer is prepared. If
it returns something else or if it was allowed to sleep then it is ignored.
Let iscsit_check_session_usage_count() return true if it needs to arm the
timer - otherwise false. This simplifies the code flow of the only caller.

iov_iter_bvec() initialises iterators well, no need to pre-zero it
beforehand as done in fd_execute_rw_aio(). Compilers can't optimise it out
and generate extra code for that (confirmed with assembly).

When attempting to match EXTENDED COPY CSCD descriptors with corresponding
se_devices, target_xcopy_locate_se_dev_e4() currently iterates over LIO's
global devices list which includes all configured backstores.
This change ensures that only initiator-accessible backstores are
considered during CSCD descriptor lookup, according to the session's
se_node_acl LUN list.
To avoid LUN removal race conditions, device pinning is changed from being
configfs based to instead using the se_node_acl lun_ref.

Now that the hd_struct always has a block device attached to it, there is
no need for having two size field that just get out of sync.
Additionally the field in hd_struct did not use proper serialization,
possibly allowing for torn writes. By only using the block_device field
this problem also gets fixed.

Switch the block device lookup interfaces to directly work with a dev_t
so that struct block_device references are only acquired by the
blkdev_get variants (and the blk-cgroup special case). This means that
we now don't need an extra reference in the inode and can generally
simplify handling of struct block_device to keep the lookups contained
in the core block layer code.

Maurizio found a race where the abort and cmd stop paths can race as
follows:
1. thread1 runs iscsit_release_commands_from_conn and sets
CMD_T_FABRIC_STOP.
2. thread2 runs iscsit_aborted_task and then does __iscsit_free_cmd. It
then returns from the aborted_task callout and we finish
target_handle_abort and do:
target_handle_abort -> transport_cmd_check_stop_to_fabric ->
lio_check_stop_free -> target_put_sess_cmd
The cmd is now freed.
3. thread1 now finishes iscsit_release_commands_from_conn and runs
iscsit_free_cmd while accessing a command we just released.
In __target_check_io_state we check for CMD_T_FABRIC_STOP and set the
CMD_T_ABORTED if the driver is not cleaning up the cmd because of a session
shutdown. However, iscsit_release_commands_from_conn only sets the
CMD_T_FABRIC_STOP and does not check to see if the abort path has claimed
completion ownership of the command.
This adds a check in iscsit_release_commands_from_conn so only the abort or
fabric stop path cleanup the command.

Do a state_list/execute_task_lock per CPU, so we can do submissions from
different CPUs without contention with each other.
Note: tcm_fc was passing TARGET_SCF_USE_CPUID, but never set cpuid. The
assumption is that it wanted to set the cpuid to the CPU it was submitting
from so it will get this behavior with this patch.

percpu_ref_init sets the refcount to 1 and percpu_ref_kill drops it.
Drivers like iSCSI and loop do not call target_sess_cmd_list_set_waiting
during session shutdown, though, so they have been calling percpu_ref_exit
with a refcount still taken and leaking the cmd_counts memory.

Fix two bugs in the LUN refcounting:
1. For the TCM_WRITE_PROTECTED case we were returning an error after
taking a ref to the LUN, but never dropping it (caller just send status
and drops cmd ref).
2. We still need to do a percpu_ref_tryget_live for the virt LUN 0 like we
do for other LUNs, because the TPG code does the refcount/wait process
like it does with other LUNs.

SBC-4 r15 5.3 COMPARE AND WRITE command states:
if the compare operation does not indicate a match, then terminate the
command with CHECK CONDITION status with the sense key set to
MISCOMPARE and the additional sense code set to MISCOMPARE DURING
VERIFY OPERATION. In the sense data (see 4.18 and SPC-5) the offset
from the start of the Data-Out Buffer to the first byte of data that
was not equal shall be reported in the INFORMATION field.
This change implements the missing logic to report the miscompare offset in
the sense data INFORMATION field. As an optimization, byte-by-byte
miscompare offset calculation is only performed after memcmp() mismatch.

cmd.bad_sector currently gets packed into the sense INFORMATION field for
TCM_LOGICAL_BLOCK_{GUARD,APP_TAG,REF_TAG}_CHECK_FAILED errors, which carry
an .add_sector_info flag in the sense_detail_table to ensure this.
In preparation for propagating a byte offset on COMPARE AND WRITE
TCM_MISCOMPARE_VERIFY error, rename cmd.bad_sector to cmd.sense_info and
sense_detail.add_sector_info to sense_detail.add_sense_info so that it
better reflects the sense INFORMATION field destination.

scatter_data_area() and gather_data_area() are not easy to understand since
data is copied in nested loops over sg_list and tcmu dbi list. Since sg
list can contain only partly filled pages, the loop has to be prepared to
handle sg pages not matching dbi pages one by one.
Existing implementation uses kmap_atomic()/kunmap_atomic() due to
performance reasons. But instead of using these calls strictly nested for
sg and dpi pages, the code holds the mappings in an overlapping way, which
indeed is a bug that would trigger on archs using highmem.
The scatterlist lib contains the sg_miter_start/_next/_stop functions which
can be used to simplify such complicated loops.
The new code now processes the dbi list in the outer loop, while sg list is
handled by the inner one. That way the code can take advantage of the
sg_miter_* family calls.
Calling sg_miter_stop() after the end of the inner loop enforces strict
nesting of atomic kmaps.
Since the nested loops in scatter_/gather_data_area were very similar, I
replaced them by the new helper function tcmu_copy_data().

The set of core changes here is Christoph's submission path cleanups.
These introduced a couple of regressions when first proposed so they
got held over from the initial merge window pull request to give more
testing time, which they've now had and Syzbot has confirmed the
regression it detected is fixed. The other main changes are two
driver updates (arcmsr, pm80xx) and assorted minor clean ups.

scatter_data_area() has two purposes:
1) Create the iovs for the data area buffer of a SCSI cmd.
2) If there is data in DMA_TO_DEVICE direction, copy
the data from sg_list to data area buffer.
Both are done in a common loop.
In case of DMA_FROM_DEVICE data transfer, scatter_data_area() is called
with parameter copy_data = false. But this flag is just used to skip
memcpy() for data, while radix_tree_lookup still is called for every dbi of
the area area buffer, and kmap and kunmap are called for every page from
sg_list and data_area as well as flush_dcache_page() for the data area
pages. Since the only thing to do with copy_data = false would be to set
up the iovs, this is a noticeable overhead. Rework the iov creation in the
main loop of scatter_data_area() providing the new function
new_block_to_iov(). Based on this, create the short new function
tcmu_setup_iovs() that only writes the iovs with no overhead. This new
function is now called instead of scatter_data_area() for bidi buffers and
for data buffers in those cases where memcpy() would have been skipped.

queue_cmd_ring() needs to check whether there is enough space in cmd ring
and data area for the cmd to queue.
Currently the sequence is:
1) Calculate size the cmd will occupy on the ring based on estimation of
needed iovs.
2) Check whether there is enough space on the ring based on size from 1)
3) Allocate buffers in data area.
4) Calculate number of iovs the command really needs while copying
incoming data (if any) to data area.
5) Re-calculate real size of cmd on ring based on real number of iovs.
6) Set up possible padding and cmd on the ring.
Step 1) must not underestimate the cmd size so use max possible number of
iovs for the given I/O data size. The resulting overestimation can be
really high so this sequence is not ideal. The earliest the real number of
iovs can be calculated is after data buffer allocation. Therefore rework
the code to implement the following sequence:
A) Allocate buffers on data area and calculate number of necessary iovs
during this.
B) Calculate real size of cmd on ring based on number of iovs.
C) Check whether there is enough space on the ring.
D) Set up possible padding and cmd on the ring.
The new sequence enforces the split of new function tcmu_alloc_data_space()
from is_ring_space_avail(). Using this function, change queue_cmd_ring()
according to the new sequence.
Change routines called by tcmu_alloc_data_space() to allow calculating and
returning the iov count. Remove counting of iovs in scatter_data_area().

Simplify code by joining tcmu_cmd_get_data_length() and
tcmu_cmd_get_block_cnt() into tcmu_cmd_set_block_cnts(). The new function
sets tcmu_cmd->dbi_cnt and also the new field tcmu_cmd->dbi_bidi_cnt which
is needed for further enhancements in following patches. Simplify some
code by using tcmu_cmd->dbi(_bidi)_cnt instead of calculation from length.
Please note: The calculation of the number of dbis needed for bidi was
wrong. It was based on the length of the first bidi sg only. I changed it
to correctly sum up entire length of all bidi sgs.

This can be reproduced via the following steps:
1. Initiator A tries to log in to iqn1-tpg1 on port 3260. After finishing
PDU exchange in the login thread and before the negotiation is finished
the the network link goes down. At this point A has not finished login
and tpg->np_login_sem is held.
2. Initiator B tries to log in to iqn2-tpg1 on port 3260. After finishing
PDU exchange in the login thread the target expects to process remaining
login PDUs in workqueue context.
3. Initiator A' tries to log in to iqn1-tpg1 on port 3260 from a new
socket. A' will wait for tpg->np_login_sem with np->np_login_timer
loaded to wait for at most 15 seconds. The lock is held by A so A'
eventually times out.
4. Before A' got timeout initiator B gets negotiation failed and calls
iscsi_target_login_drop()->iscsi_target_login_sess_out(). The
np->np_login_timer is canceled and initiator A' will hang forever.
Because A' is now in the login thread, no new login requests can be
serviced.
Fix this by moving iscsi_stop_login_thread_timer() out of
iscsi_target_login_sess_out(). Also remove iscsi_np parameter from
iscsi_target_login_sess_out().

As said by Linus:
A symmetric naming is only helpful if it implies symmetries in use.
Otherwise it's actively misleading.
In "kzalloc()", the z is meaningful and an important part of what the
caller wants.
In "kzfree()", the z is actively detrimental, because maybe in the
future we really _might_ want to use that "memfill(0xdeadbeef)" or
something. The "zero" part of the interface isn't even _relevant_.
The main reason that kzfree() exists is to clear sensitive information
that should not be leaked to other future users of the same memory
objects.
Rename kzfree() to kfree_sensitive() to follow the example of the recently
added kvfree_sensitive() and make the intention of the API more explicit.
In addition, memzero_explicit() is used to clear the memory to make sure
that it won't get optimized away by the compiler.
The renaming is done by using the command sequence:

Add "tmr_notification" configfs attribute to tcmu devices. If the default
value 0 is used, tcmu only removes aborted commands from qfull_queue. If
user changes tmr_notification to 1, additionally TMR notifications will be
written to the cmd ring.

This patch implements the tmr_notify callback for tcmu. When the callback
is called, tcmu checks the list of aborted commands it received as
parameter:
- aborted commands in the qfull_queue are removed from the queue and
target_complete_command is called
- from the cmd_ids of aborted commands currently uncompleted in cmd ring
it creates a list of aborted cmd_ids.
Finally a TMR notification is written to cmd ring containing TMR type and
cmd_id list. If there is no space in ring, the TMR notification is queued
on a TMR specific queue.
The TMR specific queue 'tmr_queue' can be seen as a extension of the cmd
ring. At the end of each iexecution of tcmu_complete_commands() we check
whether tmr_queue contains TMRs and try to move them onto the ring. If
tmr_queue is not empty after that, we don't call run_qfull_queue() because
commands must not overtake TMRs.
This way we guarantee that cmd_ids in TMR notification received by
userspace either match an active, not yet completed command or are no
longer valid due to userspace having complete some cmd_ids meanwhile.
New commands that were assigned to an aborted cmd_id will always appear on
the cmd ring _after_ the TMR.

During cmd timeout handling in check_timedout_devices(), due to a race, it
can happen that tcmu_set_next_deadline() does not start a timer as
expected:
1) Either tcmu_check_expired_ring_cmd() checks the inflight_queue or
tcmu_check_expired_queue_cmd() checks the qfull_queue while jiffies has
the value X
2) At the end of the check the queue contains one remaining command with
deadline X (time_after(X, X) is false and thus the command is not
handled as being timed out).
3) After tcmu_check_expired_xxxxx_cmd() a timer interrupt happens and
jiffies is incremented to X+1.
4) Now tcmu_set_next_deadline() is called, but it skips the command, since
time_after(X+1, X) is true. Therefore tcmu_set_next_deadline() finds no
new deadline and stops the timer, which it shouldn't.
Since commands that time out are removed from inflight_queue or
qfull_queue, we don't need the check with time_after() in
tcmu_set_next_deadline() but can use the deadline from the first cmd in
the queue.
Additionally, replace the remaining time_after() calls in
tcmu_check_expired_xxxxx_cmd() with time_after_eq(), because it is not
useful to set the timeout to deadline but then check for jiffies being
greater than deadline.
Simplify the end of tcmu_handle_completions() and change the check for no
more pending commands from
mb->cmd_tail == mb->cmd_head
to
idr_is_empty(&udev->commands)
because the old check doesn't work correctly if paddings or in the future
TMRs are in the ring.
Finally tcmu_set_next_deadline() was shifted in the source as

The new helper ring_insert_padding is split off from and then called by
queue_cmd_ring. It inserts a padding if necessary. The new helper will in
a subsequent patch be used during writing of TMR notifications to command
ring.

Target core is modified to call an optional backend callback function if a
TMR is received or commands are aborted implicitly after a PR command was
received. The backend function takes as parameters the se_dev, the type of
the TMR, and the list of aborted commands. If no commands were aborted, an
empty list is supplied.

This patch modifies core_tmr_abort_task() to use same looping and locking
scheme as core_tmr_drain_state_list() does.
This frees the state_list element in se_cmd for later use by tmr
notification handling.
Note: __target_check_io_state() now is called with param 0 instead of
dev->dev_attrib.emulate_tas, because tas is not relevant since we always
get ABRT on same session like the aborted command.

iscsi_target_sk_data_ready() could be invoked indirectly by
iscsi_target_do_login_rx() from the workqueue like this:

At that time LOGIN_FLAGS_READ_ACTIVE is not cleared and
iscsi_target_sk_data_ready will not read data from the socket. Some iscsi
initiators (libiscsi) will wait forever for a reply.
LOGIN_FLAGS_READ_ACTIVE should be cleared early just after doing the
receive and before writing to the socket in iscsi_target_do_login_rx.
Unfortunately, LOGIN_FLAGS_READ_ACTIVE is also used by sk_state_change to
do login cleanup if a socket was closed at login time. It is supposed to be
cleared after the login PDU is successfully processed and replied.
Introduce another flag, LOGIN_FLAGS_WRITE_ACTIVE, to cover the transmit
part.

SPC4 has:
The first ISCSI INITIATOR SESSION ID field byte containing an ASCII null
character terminates the ISCSI INITIATOR SESSION ID field without regard
for the specified length of the iSCSI TransportID or the contents of the
ADDITIONAL LENGTH field.

which sounds like we can get an iSID shorter than 12 chars. SPC and the
iSCSI RFC do not say how to handle that case other than just cutting off
the iSID. This patch just makes sure that if we get an iSID like that, we
only copy/send that string.
There is no OS that does this right now, so there was no test case. I did
test with sg utils to check it works as expected and nothing breaks.

This fixes the following bugs with the transport id setup for iscsi:
1. Incorrectly adding NULL after initiator name for TPID format 1.
2. For TPID format 1 buffer setup we are doing off+len, off++ and then
also len+=some_value. This results in the isid going past buffer
boundaries when we then do buf[off+len]
3. The pr_reg_isid is the isid in string format which is 12 bytes, but we
are only copying 6 bytes.

The length passed in the ADDITIONAL LENGTH field includes padding and the
terminating NULL for the last field (name or isid depending on the format),

__core_scsi3_add_registration clears the t10_pr_registration pr_reg_deve
and does a core_scsi3_lunacl_undepend_item which does an undepend and also
does a kref_put from the get done in __core_scsi3_alloc_registration. So
when we get to the bottom of core_scsi3_decode_spec_i_port the pr_reg_deve
is NULL and we crash when trying to access the local_pr_reg's pr_reg_deve.
We've also done an extra undepend for local_pr_reg and if we didn't crash
on the NULL we would have done an extra kref_put too.
This patch has us do a core_scsi3_lunacl_depend_item for local_pr_reg and
then let __core_scsi3_add_registration handle the cleanup for the
pr_reg_deve. We then just skip the undepend for the acl and tpg for the
local pr_reg.
The error path then works in a similar way, but we always do the
core_scsi3_lunacl_undepend_item since we never call
__core_scsi3_add_registration in that code path.

transport_init_session can allocate memory via percpu_ref_init, and
target_xcopy_release_pt never frees it. This adds a
transport_uninit_session function to handle cleanup of resources allocated
in the init function.

Move the check for enforce_pr_isids to the registration code where we can
fail at the time an initiator tries to register a path without an isid. In
its current place in __core_scsi3_locate_pr_reg, it is too late because it
can be registered and be reported in PR in commands and it is stuck in this
state because we cannot unregister it.

If tcmu_handle_completions() has to process a padding shorter than
sizeof(struct tcmu_cmd_entry), the current call to
tcmu_flush_dcache_range() with sizeof(struct tcmu_cmd_entry) as length
param is wrong and causes crashes on e.g. ARM, because
tcmu_flush_dcache_range() in this case calls
flush_dcache_page(vmalloc_to_page(start)); with start being an invalid
address above the end of the vmalloc'ed area.
The fix is to use the minimum of remaining ring space and sizeof(struct
tcmu_cmd_entry) as the length param.
The patch was tested on kernel 4.19.118.

(scatter|gather)_data_area() need to flush dcache after writing data to or
before reading data from a page in uio data area. The two routines are
able to handle data transfer to/from such a page in fragments and flush the
cache after each fragment was copied by calling the wrapper
tcmu_flush_dcache_range().
That means:
1) flush_dcache_page() can be called multiple times for the same page.
2) Calling flush_dcache_page() indirectly using the wrapper does not make
sense, because each call of the wrapper is for one single page only and
the calling routine already has the correct page pointer.
Change (scatter|gather)_data_area() such that, instead of calling
tcmu_flush_dcache_range() before/after each memcpy, it now calls
flush_dcache_page() before unmapping a page (when writing is complete for
that page) or after mapping a page (when starting to read the page).
After this change only calls to tcmu_flush_dcache_range() for addresses in
vmalloc'ed command ring are left over.
The patch was tested on ARM with kernel 4.19.118 and 5.7.2

NULL pointer dereference happens when the following conditions are met:
1) A SCSI command is received for a non-existing LU or cdb initialization

initialized and hence can be referenced in trace_target_cmd_complete().

Initialization of orig_fe_lun is moved to transport_init_se_cmd() from
transport_lookup_cmd_lun(). This helps for the cases where the SCSI request
fails before the call to transport_lookup_cmd_lun() so that
trace_target_cmd_complete() can print the LUN information to the trace
buffer. Due to this change, the lun parameter is removed from
transport_lookup_cmd_lun() and transport_lookup_tmr_lun().

1) If remaining ring space before the end of the ring is smaller then the
next cmd to write, tcmu writes a padding entry which fills the remaining
space at the end of the ring.
Then tcmu calls tcmu_flush_dcache_range() with the size of struct
tcmu_cmd_entry as data length to flush. If the space filled by the
padding was smaller then tcmu_cmd_entry, tcmu_flush_dcache_range() is
called for an address range reaching behind the end of the vmalloc'ed
ring.
tcmu_flush_dcache_range() in a loop calls
flush_dcache_page(virt_to_page(start)); for every page being part of the
range. On x86 the line is optimized out by the compiler, as
flush_dcache_page() is empty on x86.
But I assume the above can cause trouble on other architectures that
really have a flush_dcache_page(). For paddings only the header part of
an entry is relevant due to alignment rules the header always fits in
the remaining space, if padding is needed. So tcmu_flush_dcache_range()
can safely be called with sizeof(entry->hdr) as the length here.
2) After it has written a command to cmd ring, tcmu calls
tcmu_flush_dcache_range() using the size of a struct tcmu_cmd_entry as
data length to flush. But if a command needs many iovecs, the real size
of the command may be bigger then tcmu_cmd_entry, so a part of the
written command is not flushed then.

Add a helper to directly set the SO_REUSEADDR sockopt from kernel space
without going through a fake uaccess.
For this the iscsi target now has to formally depend on inet to avoid
a mostly theoretical compile failure. For actual operation it already
did depend on having ipv4 or ipv6 support.

The pr_debug() dereferences "cmd" after we already freed it by calling
tcmu_free_cmd(cmd). The debug printk needs to be done earlier.

When tcmu queues a new command - no matter whether in command ring or in
qfull_queue - a cmd_id from IDR udev->commands is assigned to the command.
If userspace sends a wrong command completion containing the cmd_id of a
command on the qfull_queue, tcmu_handle_completions() finds the command in
the IDR and calls tcmu_handle_completion() for it. This might do some nasty
things because commands in qfull_queue do not have a valid dbi list.
To fix this bug, we no longer add queued commands to the idr. Instead the
cmd_id is assign when a command is written to the command ring.
Due to this change I had to adapt the source code at several places where
up to now an idr_for_each had been done.

The NON_EXISTENT_LUN error can be written without an error condition
on the initiator responsible. Adding the initiatorname to this message
will reduce the effort required to fix this when many initiators are
supported by a target.
This version ensures the initiator name is also printed on the same message
in transport_lookup_tmr_lun for consistency.

Testing with Loopback I found that, after a Loopback LUN has executed a
TMR, I can no longer unlink the LUN. The rm command hangs in
transport_clear_lun_ref() at wait_for_completion(&lun->lun_shutdown_comp)
The reason is, that transport_lun_remove_cmd() is not called at the end of
target_tmr_work().
It seems, that in other fabrics this call happens implicitly when the
fabric drivers call transport_generic_free_cmd() during their
->queue_tm_rsp().
Unfortunately Loopback seems to not comply to the common way
of calling transport_generic_free_cmd() from ->queue_*().
Instead it calls transport_generic_free_cmd() from its
->check_stop_free() only.
But the ->check_stop_free() is called by
transport_cmd_check_stop_to_fabric() after it has reset the se_cmd->se_lun
pointer. Therefore the following transport_generic_free_cmd() skips the
transport_lun_remove_cmd().
So this patch re-adds the transport_lun_remove_cmd() at the end of
target_tmr_work(), which was removed during commit 2c9fa49e10 ("scsi:
target/core: Make ABORT and LUN RESET handling synchronous").
For fabrics using transport_generic_free_cmd() in the usual way the double
call to transport_lun_remove_cmd() doesn't harm, as
transport_lun_remove_cmd() checks for this situation and does not release
lun_ref twice.

We use tcm_loop with tape emulations running on tcmu.
In case application reads a short tape block with a longer READ, or a long
tape block with a short READ, according to SCC spec data has to be
tranferred _and_ sensebytes with ILI set and information field containing
the residual count. Similar problem also exists when using fixed block
size in READ.
Up to now tcm_loop is not prepared to handle sensebytes if input data is
provided, as in tcm_loop_queue_data_in() it only sets SAM_STAT_GOOD and, if
necessary, the residual count.
To fix the bug, the same handling for sensebytes as present in
tcm_loop_queue_status() must be done in tcm_loop_queue_data_in() also.
After adding this handling, the two function now are nearly identical, so I
created a single function with two wrappers.

Currently in tcmu reservation commands are handled by core's pr
implementation (default) or completely rejected (emulate_pr set to 0). We
additionally want to be able to do full reservation handling in
userspace. Therefore we need a way to set TRANSPORT_FLAG_PASSTHROUGH_PGR.
The inverted flag is displayed by attribute pgr_support. Since we moved
the flag from transport/backend to se_device in the previous commit, we now
can make it changeable per device by allowing to write the attribute. The
new field transport_flags_changeable in transport/backend is used to reject
writing if not allowed for a backend.
Regarding ALUA we also want to be able to passthrough commands to userspace
in tcmu. Therefore we need TRANSPORT_FLAG_PASSTHROUGH_ALUA to be
changeable, because by setting it we can switch off all ALUA checks in
core. So we also set TRANSPORT_FLAG_PASSTHROUGH_ALUA in tcmu's
transport_flags_changeable.
Of course, ALUA and reservation handling in userspace will work only, if
session/nexus information is sent to userspace along with every
command. This will be object of a patch series announced by Mike Christie.

pgr_support and alua_support device attributes show the inverted value of
the transport_flags:
* TRANSPORT_FLAG_PASSTHROUGH_PGR
* TRANSPORT_FLAG_PASSTHROUGH_ALUA
These attributes are per device, while the flags are per backend. Rename
the transport_flags in backend/transport to transport_flags_default and use
this value to initialize the new transport_flags field in the se_device
structure.
Now data and attribute both are per se_device.
