mm,hwpoison: refactor soft_offline_huge_page and __soft_offline_page
Merging soft_offline_huge_page and __soft_offline_page let us get rid of quite some duplicated code, and makes the code much easier to follow. Now, __soft_offline_page will handle both normal and hugetlb pages.
