READ/WRITE proved to be actively confusing - the meanings are
"data destination, as used with read(2)" and "data source, as
used with write(2)", but people keep interpreting those as
"we read data from it" and "we write data to it", i.e. exactly
the wrong way.
Call them ITER_DEST and ITER_SOURCE - at least that is harder
to misinterpret...

While handling an I/O completion for the compare portion of a
COMPARE_AND_WRITE command, it may happen that the
compare_and_write_callback function submits new bio structs while still in
softirq context.
Low level drivers like md raid5 do not expect their make_request call to be
used in softirq context, they call into schedule() and create a deadlocked
system.

This problem appears to be an issue between target_cmd_interrupted() and
compare_and_write_callback(). target_cmd_interrupted() calls the se_cmd's
transport_complete_callback function pointer if the se_cmd is being stopped
or aborted, and CMD_T_ABORTED was set on the se_cmd.
When calling compare_and_write_callback(), the success parameter was set to
false. target_cmd_interrupted() seems to expect this means the callback
will do cleanup that does not require a process context. But
compare_and_write_callback() ignores the parameter if there was I/O done
for the compare part of COMPARE_AND_WRITE.
Since there was data, the function continued on, passed the compare, and
issued a write while ignoring the value of the success parameter. The
submit of a bio for the write portion of the COMPARE_AND_WRITE then causes
schedule to be unsafely called from the softirq context.
Fix the bug in compare_and_write_callback by jumping to the out label if
success == "false", after checking if we have been called by
transport_generic_request_failure(); The command is being aborted or
stopped so there is no need to submit the write bio for the write part of
the COMPARE_AND_WRITE command.

In case a malicious initiator sends some random data immediately after a
login PDU; the iscsi_target_sk_data_ready() callback will schedule the
login_work and, at the same time, the negotiation may end without clearing
the LOGIN_FLAGS_INITIAL_PDU flag (because no additional PDU exchanges are
required to complete the login).
The login has been completed but the login_work function will find the
LOGIN_FLAGS_INITIAL_PDU flag set and will never stop from rescheduling
itself; at this point, if the initiator drops the connection, the
iscsit_conn structure will be freed, login_work will dereference a released
socket structure and the kernel crashes.

To determine how many blocks sends in one command, the minimum value is
selected from the hw_max_sectors of both devices. In target_xcopy_do_work,
hw_max_sectors are used as blocks, not sectors; it also ignores the fact
that sectors can be of different sizes, for example 512 and 4096
bytes. Because of this, a number of blocks can be transmitted that the
device will not be able to accept.
Change the selection of max transmission size into bytes.

By default, hw_max_sectors stores its value in 512 blocks in iblock,
despite the fact that the block size can be 4096 bytes. Change
hw_max_sectors to store the number of sectors in hw_block_size blocks.

A MAXIMUM TRANSFER LENGTH value indicates the maximum transfer length in
logical blocks that the device server accepts for a single command. Fix
function sending the length in sectors instead of blocks.
This patch also removes the special casing for fileio in block_size_store
since this logic in now unified in spc_emulate_evpd_b0() for all backends.

If device_register() fails in tcm_loop_setup_hba_bus(), the name allocated
by dev_set_name() need be freed. As comment of device_register() says, it
should use put_device() to give up the reference in the error path. So fix
this by calling put_device(), then the name can be freed in kobject_cleanup().
The 'tl_hba' will be freed in tcm_loop_release_adapter(), so it don't need
goto error label in this case.

The prandom_u32() function has been a deprecated inline wrapper around
get_random_u32() for several releases now, and compiles down to the
exact same code. Replace the deprecated wrapper with a direct call to
the real function. The same also applies to get_random_int(), which is
just a wrapper around get_random_u32(). This was done as a basic find
and replace.

Updates to the usual drivers (qla2xxx, lpfc, ufs, hisi_sas, mpi3mr,
mpt3sas, target); the biggest change (from my biased viewpoint) being
that the mpi3mr now attached to the SAS transport class, making it the
first fusion type device to do so. Beyond the usual bug fixing and
security class reworks, there aren't a huge number of core changes.

Match a key only if SARK is not zero according to SPC-4 and the comment
above the code:
If an all registrants persistent reservation is present and the SERVICE
ACTION RESERVATION KEY field is set to zero, then all registrations
shall be removed except for that of the I_T nexus that is being used
for the PERSISTENT RESERVE OUT command;

The default target port group is always returned in the list of port
groups, even if the behaviour is unwanted, i.e. it has no members and
non-default port groups are primary port groups.
That violates SPC-4 "6.37 REPORT TARGET PORT GROUPS command":
Every target port group shall contain at least one target port. The
target port group descriptor shall include one target port descriptor for
each target port in the target port group.
This patch hides port groups with no ports in REPORT TARGET PORT GROUPS
response.

SAM-5 4.8.3 (SCSI target device with multiple SCSI ports structure)
obligates to set MULTIP bit when there's multiple SCSI target ports:
Each device server shall indicate the presence of multiple SCSI
target ports by setting the MULTIP bit to one in its standard
INQUIRY data (see SPC-4).
Set MULTIP bit automatically to indicate the presence of multiple SCSI
target ports within standard inquiry response data if there are
multiple target ports in all target port groups of the se_device.

We had historically not checked that genlmsghdr.reserved
is 0 on input which prevents us from using those precious
bytes in the future.
One use case would be to extend the cmd field, which is
currently just 8 bits wide and 256 is not a lot of commands
for some core families.
To make sure that new families do the right thing by default
put the onus of opting out of validation on existing families.

Despite usage of RCU primitives with deve->se_lun pointer, it has not
become dereference-safe because deve->se_lun is updated and not
synchronized with a reader. That change might be in a release function
called by synchronize_rcu(). But, in fact, there is no point in setting
that pointer to NULL for deleting deve. All access to deve->se_lun is
already under rcu_read_lock. And either deve->se_lun is always valid or
deve is not valid itself and will not be found in the list_for_*. The same
applicable for deve->se_lun_acl too. So a better solution is to remove
that NULLing.

This patch removes XDWRITEREAD support because it never fully worked when
it was added in the initial LIO merge and it's been fully broken since 2013
from commit a289008749 ("target: Add compare_and_write_post() completion
callback fall through").
The two issues above are:
1. XDWRITEREAD support was just never completed when LIO was merged. We
never did the DISABLE WRITE check and so we never write data out.
2. Since the commit above, we never complete the command. After we do the
XOR, we return from xdreadwrite_callback and that's it. We never send a
response for the command, so the command will always time out and fail.
Since this has been fully broken for almost nine years this patch just
removes emulated support.

RFC7143 states that Initiator decides what type of authentication to
use:
The initiator MUST continue with:
CHAP_N=<N> CHAP_R=<R>
or, if it requires target authentication, with:
CHAP_N=<N> CHAP_R=<R> CHAP_I=<I> CHAP_C=<C>
Allow one way authentication if mutual authentication is configured. That
passes some tests from Windows HLK for Mutual CHAP with iSNS.

RFC7143 allows both Base64 and Hex encoding for CHAP binary entities like
Challenge and Response. Currently the Linux iSCSI target supports only Hex
encoding.
Add support for Base64 encoded CHAP Challenge and CHAP Response required
for CHAP tests in Windows HLK.

Variable length SCSI commands are transferred over iSCSI via two CDB
buffers - in Basic Header Segment and in Additional Header Segment (AHS).
Since AHS is not supported yet, a target reads just BHS (48 byte) from TCP
and treats the remaining octets as a next new iSCSI PDU that causes
protocol errors.
Add support for the Extended CDB AHS type.

On our backend we can do something similar to LIO where we can enable and
disable UNMAP support on the fly. In the SCSI/block layer we can detect
this by just doing a rescan. However, LIO cannot detect this change because
we only check during the initial configuration. This patch allows UNMAP
detection to also happen when the user tries to turn it on.

We use WSNZ=1 so if we get a WRITE_SAME with zero logical blocks we are
supposed to fail it. We do this check and failure in target_core_sbc.c
before calling into the backend, so we can remove the incorrect check in
target_core_file.

In newer version of the SBC specs, we have a NDOB bit that indicates there
is no data buffer that gets written out. If this bit is set using commands
like "sg_write_same --ndob" we will crash in target_core_iblock/file's
execute_write_same handlers when we go to access the se_cmd->t_data_sg
because its NULL.
This patch adds a check for the NDOB bit in the common WRITE SAME code
because we don't support it. And, it adds a check for zero SG elements in
each handler in case the initiator tries to send a normal WRITE SAME with
no data buffer.

Add acls/{ACL}/attrib/authentication attribute that controls authentication
for particular ACL. By default, this attribute inherits a value of the
authentication attribute of the target port group to keep backward
compatibility.
Authentication attribute has 3 states:
"0" - authentication is turned off for this ACL
"1" - authentication is required for this ACL
"-1" - authentication is inherited from TPG

iSCSI target is cluttered with open-coded container_of() conversions from
se_nacl to iscsi_node_acl. The code could be cleaned by introducing a
helper - to_iscsi_nacl() (similar to other helpers in target core).
While at it, make another iSCSI conversion helper consistent and rename
iscsi_tpg() helper to to_iscsi_tpg().

In tcmu_blocks_release(), lock_page() is called to prevent a race causing
possible data corruption. Since lock_page() might sleep, calling it while
holding XArray lock is a bug.
To fix this, replace the xas_for_each() call with xa_for_each_range().
Since the latter does its own handling of XArray locking, the xas_lock()
and xas_unlock() calls around the original loop are no longer necessary.
The switch to xa_for_each_range() slows down the loop slightly. This is
acceptable since tcmu_blocks_release() is not relevant for performance.

When tcmu_vma_fault() gets a page successfully, before the current context
completes page fault procedure, find_free_blocks() may run and call
unmap_mapping_range() to unmap the page. Assume that when
find_free_blocks() initially completes and the previous page fault
procedure starts to run again and completes, then one truncated page has
been mapped to userspace. But note that tcmu_vma_fault() has gotten a
refcount for the page so any other subsystem won't be able to use the page
unless the userspace address is unmapped later.
If another command subsequently runs and needs to extend dbi_thresh it may
reuse the corresponding slot for the previous page in data_bitmap. Then
though we'll allocate new page for this slot in data_area, no page fault
will happen because we have a valid map and the real request's data will be
lost.
Filesystem implementations will also run into this issue but they usually
lock the page when vm_operations_struct->fault gets a page and unlock the
page after finish_fault() completes. For truncate filesystems lock pages in
truncate_inode_pages() to protect against racing wrt. page faults.
To fix this possible data corruption scenario we can apply a method similar
to the filesystems. For pages that are to be freed, tcmu_blocks_release()
locks and unlocks. Make tcmu_vma_fault() also lock found page under
cmdr_lock. At the same time, since tcmu_vma_fault() gets an extra page
refcount, tcmu_blocks_release() won't free pages if pages are in page fault
procedure, which means it is safe to call tcmu_blocks_release() before
unmap_mapping_range().
With these changes tcmu_blocks_release() will wait for all page faults to
be completed before calling unmap_mapping_range(). And later, if
unmap_mapping_range() is called, it will ensure stale mappings are removed.

With tape devices, the SCF_TREAT_READ_AS_NORMAL flag is used by the target
subsystem to mark commands which have both data to return as well as sense
data. But with pscsi, SCF_TREAT_READ_AS_NORMAL can be set even if there is
no data to return. The SCF_TREAT_READ_AS_NORMAL flag causes the target core
to call iscsit data-in callbacks even if there is no data, which iscsit
does not support. This results in iscsit going into an error state
requiring recovery and being unable to complete the command to the
initiator.
This issue can be resolved by fixing pscsi to only set
SCF_TREAT_READ_AS_NORMAL if there is valid data to return alongside the
sense data.

Target does not support some VPD pages and is very verbose about it.
Sometimes initiators don't bother and just keep sending the same request
from time to time, filling up the logs.
This patch lowers the message priority to debug.

Secure erase is a very different operation from discard in that it is
a data integrity operation vs hint. Fully split the limits and helper

Just use a non-zero max_discard_sectors as an indicator for discard
support, similar to what is done for write zeroes.
The only places where needs special attention is the RAID5 driver,
which must clear discard support for security reasons by default,
even if the default stacking rules would allow for it.

Add a helper to query the number of sectors support per each discard bio
based on the block device and use this helper to stop various places from
poking into the request_queue to see if discard is supported and if so how
much. This mirrors what is done e.g. for write zeroes as well.

For block devices, the SCSI target drivers implements UNMAP as calls to
blkdev_issue_discard, which does not guarantee zeroing just because
Write Zeroes is supported.
Note that this does not affect the file backed path which uses
fallocate to punch holes.

The target driver prevents the users from changing the database root
directory if a target module like ib_srpt has been registered. This makes
it difficult for users to set their preferred database directory if the
module gets loaded during the system boot.

tcmu_try_get_data_page() looks up pages under cmdr_lock, but it does not
take refcount properly and just returns page pointer. When
tcmu_try_get_data_page() returns, the returned page may have been freed by
tcmu_blocks_release().
We need to get_page() under cmdr_lock to avoid concurrent
tcmu_blocks_release().

The RX/TX threads for iSCSI connection can be scheduled to any online CPUs,
and will not be rescheduled.
When binding other heavy load threads along with iSCSI connection RX/TX
thread to the same CPU, the iSCSI performance will be worse.
Add iscsi/cpus_allowed_list in configfs. The available CPU set of iSCSI
connection RX/TX threads is allowed_cpus & online_cpus. If it is modified,
all RX/TX threads will be rescheduled.

Now that each scsi_request is backed by a scsi_cmnd, there is no need to
indirect the CDB storage. Change all submitters of SCSI passthrough
requests to store the CDB information directly in the scsi_cmnd, and while
doing so allocate the full 32 bytes that cover all Linux supported SCSI
hosts instead of requiring dynamic allocation for > 16 byte CDBs. On
64-bit systems this does not change the size of the scsi_cmnd at all, while
on 32-bit systems it slightly increases it for now, but that increase will
be made up by the removal of the remaining scsi_request fields.

Copy directly from the se_cmd CDB to the one in the scsi_request. This
temporarily limits the pscsi backend to supporting only up to 16 byte CDBs,
but this restriction will be lifted later in this series.

Make cmd_ring_size changeable similar to the way it is done for
max_data_area_mb. The reason is that our tcmu client will create thousands
of tcmu instances, and this will consume lots of mem with default 8Mb cmd
ring size for every backstore.
One can change the value by typing:
echo "cmd_ring_size_mb=N" > control
The "N" is a integer between 1 to 8, if set 1, the cmd ring can hold about
6k cmds(tcmu_cmd_entry about 176 byte) at least.
The value is printed when doing:
cat info
In addition, a new readonly attribute 'cmd_ring_size_mb' returns the value
in read.

iscsit_tpg_check_network_portal() has nested for_each loops and is supposed
to return true when a match is found. However, the tpg loop will still
continue after existing the tpg_np loop. If this tpg_np is not the last the
match value will be changed.
Break the outer loop after finding a match and make sure the np under each
tpg is unique.

The "item" pointer is always going to be valid pointer and does not need to
be checked.
But if "item" were NULL then item_to_lun() would not return a NULL, but
instead, the container_of() pointer math would return a value in the error
pointer range. This confuses static checkers since it looks like a NULL vs
IS_ERR() bug.
Delete the bogus checks.

Currently TMF commands are removed from de_device.dev_tmf_list at the very
end of se_cmd lifecycle. However, se_lun unlinks from se_cmd upon a command
status (response) being queued in transport layer. This means that LUN and
backend device can be deleted in the meantime and a panic will occur:

For the iSCSI protocol this is easily reproduced:
- Send some SCSI sommand
- Send Abort of that command over iSCSI
- Remove LUN on target
- Send next iSCSI command to acknowledge the Abort_Response
- Target panics
There is no need to keep the command in tmr_list until response completion,
so move the removal from tmr_list from the response completion to the
response queueing when the LUN is unlinked. Move the removal from state
list too as it is a subject to the same race condition.

The second argument was only used by the USB gadget code, yet everyone
pays the overhead of passing a zero to be passed into aio, where it
ends up being part of the aio res2 value.
Now that everybody is passing in zero, kill off the extra argument.

Add a new helper that calls blk_get_request and initializes the
scsi_request to avoid the indirect call through ->.initialize_rq_fn.
Note that this makes the pktcdvd driver depend on the SCSI core, but
given that only SCSI devices support SCSI passthrough requests that
is not a functional change.

When userspace changes the LUN's ALUA group, it will set the LUN's group to
NULL then to the new group. Before the new group is set,
target_alua_state_check() will return 0 and allow the I/O to execute. This
has us skip the NULL stage, and just swap in the new group.

We are only holding the lun_tg_pt_gp_lock in target_alua_state_check() to
make sure tg_pt_gp is not freed from under us while we copy the state,
delay, ID values. We can instead use RCU here to access the tg_pt_gp.
With this patch IOPs can increase up to 10% for jobs like:
fio --filename=/dev/sdX --direct=1 --rw=randrw --bs=4k \
--ioengine=libaio --iodepth=64 --numjobs=N
when there are multiple sessions (running that fio command to each /dev/sdX
or using multipath and there are over 8 paths), or more than 8 queues for

We can't free the tg_pt_gp in core_alua_set_tg_pt_gp_id() because it's
still accessed via configfs. Its release must go through the normal
configfs/refcount process.
The max alua_tg_pt_gps_count check should probably have been done in
core_alua_allocate_tg_pt_gp(), but with the current code userspace could
have created 0x0000ffff + 1 groups, but only set the id for 0x0000ffff.
Then it could have deleted a group with an ID set, and then set the ID for
that extra group and it would work ok.
It's unlikely, but just in case this patch continues to allow that type of
behavior, and just fixes the kfree() while in use bug.

This patch fixes the following bugs:
1. If there are multiple ordered cmds queued and multiple simple cmds
completing, target_restart_delayed_cmds() could be called on different
CPUs and each instance could start a ordered cmd. They could then run in
different orders than they were queued.
2. target_restart_delayed_cmds() and target_handle_task_attr() can race
where:
1. target_handle_task_attr() has passed the simple_cmds == 0 check.
2. transport_complete_task_attr() then decrements simple_cmds to 0.
3. transport_complete_task_attr() runs target_restart_delayed_cmds() and
it does not see any cmds on the delayed_cmd_list.
4. target_handle_task_attr() adds the cmd to the delayed_cmd_list.
The cmd will then end up timing out.
3. If we are sent > 1 ordered cmds and simple_cmds == 0, we can execute
them out of order, because target_handle_task_attr() will hit that
simple_cmds check first and return false for all ordered cmds sent.
4. We run target_restart_delayed_cmds() after every cmd completion, so if
there is more than 1 simple cmd running, we start executing ordered cmds
after that first cmd instead of waiting for all of them to complete.
5. Ordered cmds are not supposed to start until HEAD OF QUEUE and all older
cmds have completed, and not just simple.
6. It's not a bug but it doesn't make sense to take the delayed_cmd_lock
for every cmd completion when ordered cmds are almost never used. Just
replacing that lock with an atomic increases IOPs by up to 10% when
completions are spread over multiple CPUs and there are multiple
sessions/ mqs/thread accessing the same device.
This patch moves the queued delayed handling to a per device work to
serialze the cmd executions for each device and adds a new counter to track
HEAD_OF_QUEUE and SIMPLE cmds. We can then check the new counter to
determine when to run the work on the completion path.

We can race where target_handle_task_attr() has put the cmd on the
delayed_cmd_list. Then target_restart_delayed_cmds() has removed it and set
CMD_T_SENT, but then target_execute_cmd() now clears that bit.
This patch moves the clearing to before we've put the cmd on the list.

Tcmu populates the data area (used for communication with userspace) with
pages that are allocated by calling alloc_page(GFP_NOIO). Therefore
previous content of the allocated pages is exposed to user space. Avoid
this by adding __GFP_ZERO flag.
Zeroing the pages does (nearly) not affect tcmu throughput, because
allocated pages are re-used for the data transfers of later SCSI cmds.

Change the log level of the following message to debug:
Unsupported SCSI Opcode 0xXX, sending CHECK_CONDITION.
This message is mostly helpful during debugging sessions in order to
understand errors on the initiator side. But most of the time it's just
useless and makes reading logs much harder.
It gets particularly annoying if there are many initiators that come and go
or if an initiator runs a program that does not care whether the command is
supported and just keeps sending it.

Commit 356ba2a8bc ("scsi: target: tcmu: Make pgr_support and alua_support
attributes writable") introduced support for changeable alua_support and
pgr_support target attributes. These can only be changed if the backstore
is user-backed, otherwise the kernel returns -EINVAL.
This triggers a warning in the targetcli/rtslib code when performing a
target restore that includes non-userbacked backstores:

flip the PGR/ALUA bit in the transport_flags field, otherwise we will do
nothing and return success.
Return ENOSYS instead of EINVAL if the pgr/alua attributes can not be
changed, this way it will be possible for userspace to understand if the
operation failed because an invalid value has been passed to strtobool() or
because the attributes are fixed.

This series consists of the usual driver updates (ufs, qla2xxx,
target, smartpqi, lpfc, mpt3sas). The core change causing the most
churn was replacing the command request field request with a macro,
allowing us to offset map to it and remove the redundant field; the
same was also done for the tag field. The most impactful change is
the final removal of scsi_ioctl, which has been deprecated for over a
decade.

Currently, backend drivers can fail I/O with SAM_STAT_CHECK_CONDITION which
gets us TCM_LOGICAL_UNIT_COMMUNICATION_FAILURE.
Add a new helper that allows backend drivers to fail with specific sense
codes.

The return value of transport_kmap_data_sg() is assigned to the variable
buf:
buf = transport_kmap_data_sg(cmd);
And then it is checked:
if (!buf) {
This indicates that buf can be NULL. However, it is dereferenced in the
following statements:
if (!(buf[3] & 0x80))
buf[3] |= 0x80;
if (!(buf[2] & 0x80))
buf[2] |= 0x80;
To fix these possible null-pointer dereferences, dereference buf and call
transport_kunmap_data_sg() only when buf is not NULL.

The se_cmd scsi_asc and scsi_ascq members are only used for tracking ALUA
SCSI sense detail between target_core_alua and translate_sense_reason(), so
they're effectively always zero here.

core_alua_state_lba_dependent() currently uses lun->lun_tg_pt_gp without
holding the lun_tg_pt_gp_lock. The lock is taken in the caller, so obtain
the needed tg_pt_gp_id there instead.

When running command pipelining for WRITE direction commands (e.g. tape
device write), userspace sends cmd completion to cmd ring before processing
write data. In that case userspace has to copy data before sending
completion, because cmd completion also implicitly releases the data buffer
in data area.
The new feature KEEP_BUF allows userspace to optionally keep the buffer
after completion by setting new bit TCMU_UFLAG_KEEP_BUF in
tcmu_cmd_entry_hdr->uflags. In that case buffer has to be released
explicitly by writing the cmd_id to new action item free_kept_buf.
All kept buffers are released during reset_ring and if userspace closes uio
device (tcmu_release).

CONFIG_BLK_SCSI_REQUEST is rather misnamed as it enables building a small
amount of code shared by the SCSI initiator, target, and consumers of the
scsi_request passthrough API. Rename it and also allow building it as a
module.
