Set .cmd_size in the SCSI host template instead of using the SCSI pointer
from struct scsi_cmnd. This patch prepares for removal of the SCSI pointer

53555fb7bc scsi: Remove drivers/scsi/scsi.h
The following two header files have the same file name: include/scsi/scsi.h
and drivers/scsi/scsi.h. This is confusing. Remove the latter since the
following note was added in drivers/scsi/scsi.h in 2004:

"NOTE: this file only contains compatibility glue for old drivers. All
these wrappers will be removed sooner or later. For new code please use
the interfaces declared in the headers in include/scsi/"

af049dfd0b scsi: core: Remove the 'done' argument from SCSI queuecommand_lck functions
The DEF_SCSI_QCMD() macro passes the addresses of the SCSI host lock and
also that of the scsi_done function to the queuecommand_lck() function
implementations. Remove the 'scsi_done' argument since its address is
now a constant and instead call 'scsi_done' directly from inside the
queuecommand_lck() functions.

c5336400ca scsi: acornscsi: Remove scsi_cmd_to_tag() reference
Commit 756fb6a895af ("scsi: acornscsi: Remove tagged queuing vestiges")
mistakenly introduced a reference to function scsi_cmd_to_tag(). This
function does not exist as it was removed from an earlier series version
when I upstreamed the named commit - originally authored By Hannes - but
this reference still remained.

Fix by replacing the reference to scsi_cmd_to_tag() with
scsi_cmd_to_rq(scsi_scmd)->tag, which scsi_cmd_to_tag() was a wrapper for.

756fb6a895 scsi: acornscsi: Remove tagged queuing vestiges
The acornscsi driver has a config option to enable tagged queuing, but this
option gets disabled in the driver itself with the comment 'needs to be
debugged'.  As this is a _really_ old driver I doubt anyone will be wanting
to invest time here, so remove the tagged queue vestiges and make our lives
easier.

This is the set of changes collected since just before the merge
 window opened.  It's mostly minor fixes in drivers.  The one
 non-driver set is the three optical disk (sr) changes where two are
 error path fixes and one is a helper conversion.  The big driver
 change is the hpsa compat_alloc_userspace rework by Al so he can kill
 the remaining user.  This has been tested and acked by the maintainer.

4af14d113b scsi: remove the use_clustering flag
The same effects can be achieved by setting the dma_boundary to
PAGE_SIZE - 1 and the max_segment_size to PAGE_SIZE, so shift those
settings into the drivers.  Note that in many cases the setting might
be bogus, but this keeps the status quo.
