4411292267 scsi: libsas: Add sas_ata_device_link_abort()
Similar to how AHCI handles NCQ errors in ahci_error_intr() ->
ata_port_abort() -> ata_do_link_abort(), add an NCQ error handler for LLDDs
to call to initiate a link abort.

This will mark all outstanding QCs as failed and kick-off EH.

Note:

A "force reset" argument is added for drivers which require the ATA error
handling to always reset the device.

A driver may require this feature for when SATA device per-SCSI cmnd
resources are only released during reset for ATA EH. As such, we need an
option to force reset to be done, regardless of what any EH autopsy
decides.

The SATA device FIS fields are set to indicate a device error from
ata_eh_analyze_tf().

62e6e5940c SCSI misc on 20221007
Updates to the usual drivers (qla2xxx, lpfc, ufs, hisi_sas, mpi3mr,
 mpt3sas, target); the biggest change (from my biased viewpoint) being
 that the mpi3mr now attached to the SAS transport class, making it the
 first fusion type device to do so.  Beyond the usual bug fixing and
 security class reworks, there aren't a huge number of core changes.

141f3d6256 ata: libata-sata: Fix device queue depth control
The function __ata_change_queue_depth() uses the helper
ata_scsi_find_dev() to get the ata_device structure of a scsi device and
set that device maximum queue depth. However, when the ata device is
managed by libsas, ata_scsi_find_dev() returns NULL, turning
__ata_change_queue_depth() into a nop, which prevents the user from
setting the maximum queue depth of ATA devices used with libsas based
HBAs.

Fix this by renaming __ata_change_queue_depth() to
ata_change_queue_depth() and adding a pointer to the ata_device
structure of the target device as argument. This pointer is provided by
ata_scsi_change_queue_depth() using ata_scsi_find_dev() in the case of
a libata managed device and by sas_change_queue_depth() using
sas_to_ata_dev() in the case of a libsas managed ata device.

46ba53c306 scsi: libsas: Fix use-after-free bug in smp_execute_task_sg()
When executing SMP task failed, the smp_execute_task_sg() calls del_timer()
to delete "slow_task->timer". However, if the timer handler
sas_task_internal_timedout() is running, the del_timer() in
smp_execute_task_sg() will not stop it and a UAF will happen. The process
is shown below:

1e82e4627a scsi: libsas: Resume SAS host for phy reset or enable via sysfs
Currently if a phy reset or enable phy is issued via sysfs when controller
is suspended, those operations will be ignored as SAS_HA_REGISTERED is
cleared. If RPM is enabled then we may aggressively suspend automatically.
In this case it may be difficult to enable or reset a phy via sysfs, so
resume the host in these scenarios.

3dafe0648d scsi: libsas: Introduce struct smp_rps_resp
Similarly to sas report general and discovery responses, define the
structure struct smp_rps_resp to handle SATA PHY report responses using a
structure with a size that is exactly equal to the sas defined response
size.

With this change, struct smp_resp becomes unused and is removed.

Fix this by defining struct smp_rg_resp for sas report general responses.

095478a6e5 scsi: hisi_sas: Use libsas internal abort support
Use the common libsas internal abort functionality.

In addition, this driver has special handling for internal abort timeouts -
specifically whether to reset the controller in that instance, so extend
the API for that.

Timeout is now increased to 20 * Hz from 6 * Hz.

We also retry for failure now, but this should not make a difference.

5c9bf3635b scsi: libsas: Add sas_execute_internal_abort_single()
The internal abort feature is common to hisi_sas and pm8001 HBAs, and the
driver support is similar also, so add a common handler.

Two modes of operation will be supported:

 - single: Abort a single tagged command

 - device: Abort all commands associated with a specific domain device

A new protocol is added, SAS_PROTOCOL_INTERNAL_ABORT, so the common queue
command API may be re-used.

Only add "single" support as a first step.

32698c9552 scsi: libsas: Clean up sas_form_port()
Sparse throws a warning about context imbalance ("different lock contexts
for basic block") in sas_form_port() as it gets confused with the fact that
a port is locked within one of the two search loops and unlocked afterward
outside of the search loops once the phy is added to the port. Since this
code is not easy to follow, improve it by factoring out the code adding the
phy to the port once the port is locked into the helper function
sas_form_port_add_phy(). This helper can then be called directly within the
port search loops, avoiding confusion and clearing the sparse warning.

8454563e4c scsi: libsas: Fix sas_ata_qc_issue() handling of NCQ NON DATA commands
To detect for the DMA_NONE (no data transfer) DMA direction,
sas_ata_qc_issue() tests if the command protocol is ATA_PROT_NODATA.  This
test does not include the ATA_CMD_NCQ_NON_DATA command as this command
protocol is defined as ATA_PROT_NCQ_NODATA (equal to ATA_PROT_FLAG_NCQ) and
not as ATA_PROT_NODATA.

To include both NCQ and non-NCQ commands when testing for the DMA_NONE DMA
direction, use "!ata_is_data()".

Add a function to execute an ATA command using the TMF code, and use in the
hisi_sas driver. That driver needs to be able to issue the command on a
specific phy, so add an interface for that.

With that, hisi_sas_exec_internal_tmf_task() may be deleted.

001ec7f89b scsi: libsas: Add sas_execute_tmf()
Drivers using libsas have to issue their own TMFs, and the code to do this
is duplicated between drivers.

Add a common function to handle TMFs. This will be also used for sending
ATA commands, but use name "tmf" as the purpose is similar between SCSI and
ATA in the usage.

The force_phy_id argument will be used for a hisi_sas HW bug workaround.

We check the sas task status stat field against TMF codes, as according to
the SAS v1.1 spec 9.2.2.5.3, if response_data is in datapres, then the
response code should be a TMF code - see table 128.

9aacf6fe90 scsi: libsas: Handle non-TMF codes in sas_scsi_find_task()
LLDD TMF callbacks may return linux or other error codes instead of TMF
codes. This may cause problems in sas_scsi_find_task() ->
.lldd_query_task(), as only TMF codes are handled there. As such, we may
not return a task_disposition type from sas_scsi_find_task(). Function
sas_eh_handle_sas_errors() only handles that type, and will only progress
error handling for those recognised types.

Return TASK_ABORT_FAILED upon exit on the assumption that the command may
still be alive and error handling should be escalated.

307d9f49cc scsi: libsas: Keep host active while processing events
Processing events such as PORTE_BROADCAST_RCVD may cause dependency issues
for runtime power management support.  Such a problem would be that
handling a PORTE_BROADCAST_RCVD event requires that the host is resumed to
send SMP commands. However, in resuming the host, the phyup events
generated from re-enabling the phys are processed in the same workqueue as
the original PORTE_BROADCAST_RCVD event. As such, the host will never
finish resuming (as it waits for the phyup event processing), and then the
PORTE_BROADCAST_RCVD event can't be processed as the SMP commands are
blocked, and so we have a deadlock.  Solve this problem by ensuring that
libsas keeps the host active until completely finished phy or port events,
such as PORTE_BYTES_DMAED. As such, we don't have to worry about resuming
the host for processing individual SMP commands in this example.

bf19aea460 scsi: libsas: Defer works of new phys during suspend
During the processing of event PORT_BYTES_DMAED, the driver queues work
DISCE_DISCOVER_DOMAIN and then flushes workqueue ha->disco_q.  If a new
phyup event occurs during resuming the controller, the work
PORTE_BYTES_DMAED of new phy occurs before suspended phy's. The work
DISCE_DISCOVER_DOMAIN of new phy requires an active SAS controller (it
needs to resume SAS controller by function scsi_sysfs_add_sdev() and some
other functions such as function add_device_link()). However, the
activation of the SAS controller requires completion of work
PORTE_BYTES_DMAED of suspended phys while it is blocked by new phy's work
on ha->event_q. So there is a deadlock and it is released only after resume
timeout.

To solve the issue, defer works of new phys during suspend and queue those
defer works after SAS controller becomes active.

fbefe22811 scsi: libsas: Don't always drain event workqueue for HA resume
For the hisi_sas driver, if a directly attached disk is removed during
suspend, a hang will occur in the resume process:

The background is that in commit 16fd4a7c59 ("scsi: hisi_sas: Add device
link between SCSI devices and hisi_hba"), it is ensured that the HBA device
cannot be runtime suspended when any SCSI device associated is active.

Other drivers which use libsas don't worry about this as none support
runtime suspend.

The mentioned hang occurs when an disk is removed during suspend. In the
removal process - from PHYE_RESUME_TIMEOUT event processing - we call into
scsi_remove_device(), which is being processed in the HA event workqueue.
Here we wait for all suppliers of the SCSI device to resume, which includes
the HBA device (from the above commit). However the HBA device cannot
resume, as it is waiting for the PHYE_RESUME_TIMEOUT to be processed (from
calling sas_resume_ha() -> sas_drain_work()). This is the deadlock.

There does not appear to be any need for the sas_drain_work() to be called
at all in sas_resume_ha() as it is not syncing against anything, so allow
LLDDs to avoid this by providing a variant of sas_resume_ha() which does
"sync", i.e. doesn't drain the event workqueue.

49da96d779 scsi: libsas: Add LUN number check in .slave_alloc callback
Offlining a SATA device connected to a hisi SAS controller and then
scanning the host will result in detecting 255 non-existent devices:

After a REPORT LUN command issued to the offline device fails, the SCSI
midlayer tries to do a sequential scan of all devices whose LUN number is
not 0. However, SATA does not support LUN numbers at all.

Introduce a generic sas_slave_alloc() handler which will return -ENXIO for
SATA devices if the requested LUN number is larger than 0 and make libsas
drivers use this function as their .slave_alloc callback.

176ddd8917 scsi: libsas: Reset num_scatter if libata marks qc as NODATA
When the cache_type for the SCSI device is changed, the SCSI layer issues a
MODE_SELECT command. The caching mode details are communicated via a
request buffer associated with the SCSI command with data direction set as
DMA_TO_DEVICE (scsi_mode_select()). When this command reaches the libata
layer, as a part of generic initial setup, libata layer sets up the
scatterlist for the command using the SCSI command (ata_scsi_qc_new()).
This command is then translated by the libata layer into
ATA_CMD_SET_FEATURES (ata_scsi_mode_select_xlat()). The libata layer treats
this as a non-data command (ata_mselect_caching()), since it only needs an
ATA taskfile to pass the caching on/off information to the device. It does
not need the scatterlist that has been setup, so it does not perform
dma_map_sg() on the scatterlist (ata_qc_issue()). Unfortunately, when this
command reaches the libsas layer (sas_ata_qc_issue()), libsas layer sees it
as a non-data command with a scatterlist. It cannot extract the correct DMA
length since the scatterlist has not been mapped with dma_map_sg() for a
DMA operation. When this partially constructed SAS task reaches pm80xx
LLDD, it results in the following warning:

"pm80xx_chip_sata_req 6058: The sg list address
start_addr=0x0000000000000000 data_len=0x0end_addr_high=0xffffffff
end_addr_low=0xffffffff has crossed 4G boundary"

Update libsas to handle ATA non-data commands separately so num_scatter and
total_xfer_len remain 0.

f76d9f1a15 scsi: libsas: Switch back to original event notifiers API
libsas event notifiers required an extension where gfp_t flags must be
explicitly passed. For bisectability, a temporary _gfp() variant of such
functions were added. All call sites then got converted use the _gfp()
variants and explicitly pass GFP context. Having no callers left, the
original libsas notifiers were then modified to accept gfp_t flags by
default.

Switch back to the original event notifiers API, while still passing GFP
context.  The _gfp() notifier variants will be removed afterwards.

sas_alloc_event() uses in_interrupt() to decide which allocation should be
used.

The usage of in_interrupt() in drivers is phased out and Linus clearly
requested that code which changes behaviour depending on context should
either be separated or the context be conveyed in an argument passed by the
caller, which usually knows the context.

The in_interrupt() check is also only partially correct, because it fails
to choose the correct code path when just preemption or interrupts are
disabled. For example, as in the following call chain:

  mvsas/mv_sas.c: mvs_work_queue() [process context]
  spin_lock_irqsave(mvs_info::lock, )
    -> libsas/sas_event.c: sas_notify_phy_event()
      -> sas_alloc_event()
        -> in_interrupt() = false
          -> invalid GFP_KERNEL allocation
    -> libsas/sas_event.c: sas_notify_port_event()
      -> sas_alloc_event()
        -> in_interrupt() = false
          -> invalid GFP_KERNEL allocation

Introduce sas_alloc_event_gfp(), sas_notify_port_event_gfp(), and
sas_notify_phy_event_gfp(), which all behave like the non _gfp() variants
but use a caller-passed GFP mask for allocations.

For bisectability, all callers will be modified first to pass GFP context,
then the non _gfp() libsas API variants will be modified to take a gfp_t by
default.

121181f3f8 scsi: libsas: Remove notifier indirection
LLDDs report events to libsas with .notify_port_event and .notify_phy_event
callbacks.

These callbacks are fixed and so there is no reason why the functions
cannot be called directly, so do that.

This neatens the code slightly, makes it more obvious, and reduces function
pointer usage, which is generally a good thing. Downside is that there are
2x more symbol exports.

53de092f47 scsi: libsas: Set data_dir as DMA_NONE if libata marks qc as NODATA
It was discovered that sdparm will fail when attempting to disable write
cache on a SATA disk connected via libsas.

In the ATA command set the write cache state is controlled through the SET
FEATURES operation. This is roughly corresponds to MODE SELECT in SCSI and
the latter command is what is used in the SCSI-ATA translation layer. A
subtle difference is that a MODE SELECT carries data whereas SET FEATURES
is defined as a non-data command in ATA.

Set the DMA data direction to DMA_NONE if the requested ATA command is
identified as non-data.

3865337965 scsi: libsas: Check link status in ATA prereset()
libata currently attempts to reset even if the SATA disk is unplugged.  To
avoid the meaningless reset of a missing disk, libsas should report offline
status to libata. libata already provides a .prereset callback for this
purpose. This is called by ata_eh_reset() and can be used to influence
whether a reset attempt should be made.

Add sas_ata_preset callback to check status of phy and disk. If the disk is
already offline or phy is disabled, we return -ENOENT to libata to avoid
the reset.

3a243c2c35 scsi: libsas: Remove postreset from sas_sata_ops
sas_sata_ops uses ata_std_postreset as .postreset callback. However,
ata_std_postreset() calls sata_scr_read()/sata_scr_write() which need to
access the ATA SCR register. This register not available in the libsas case
and the functions always return -EOPNOTSUPP.

Drop the .postreset callback.

bd322af15c ata: make SATA_PMP option selectable only if any SATA host driver is enabled
There is no reason to expose SATA_PMP config option when no SATA
host drivers are enabled. To fix it add SATA_HOST config option,
make all SATA host drivers select it and finally make SATA_PMP
config options depend on it.

This also serves as preparation for the future changes which
optimize libata core code size on PATA only setups.

b3e3d4c618 scsi: libsas: Tidy SAS address print format
Currently we use a mixture of %016llx, %llx, and %16llx when printing a SAS
address.

Since the most significant nibble of the SAS address is always 5 - as per
standard - this formatting is not so important; but some fake SAS addresses
for SATA devices may not be. And we have mangled/invalid address to
consider also. And it's better to be consistent in the code, so use a fixed
format.

The SAS address is a fixed size at 64b, so we want to 0 byte extend to 16
nibbles, so use %016llx globally.

Also make some prints to be explicitly hex, and tidy some whitespace issue.

Note on the SPDX license tag conversion conflicts: the SCSI tree had
done its own SPDX conversion, which in some cases conflicted with the
treewide ones done by Thomas & co.

In almost all cases, the conflicts were purely syntactic: the SCSI tree
used the old-style SPDX tags ("GPL-2.0" and "GPL-2.0+") while the
treewide conversion had used the new-style ones ("GPL-2.0-only" and
"GPL-2.0-or-later").

In these cases I picked the new-style one.

In a few cases, the SPDX conversion was actually different, though.  As
explained by James above, and in more detail in a pre-pull-request
thread:

 "The other problem is actually substantive: In the libsas code Luben
  Tuikov originally specified gpl 2.0 only by dint of stating:

  * This file is licensed under GPLv2.

  In all the libsas files, but then muddied the water by quoting GPLv2
  verbatim (which includes the or later than language). So for these
  files Christoph did the conversion to v2 only SPDX tags and Thomas
  converted to v2 or later tags"

So in those cases, where the spdx tag substantially mattered, I took the
SCSI tree conversion of it, but then also took the opportunity to turn
the old-style "GPL-2.0" into a new-style "GPL-2.0-only" tag.

Similarly, when there were whitespace differences or other differences
to the comments around the copyright notices, I took the version from
the SCSI tree as being the more specific conversion.

Finally, in the spdx conversions that had no conflicts (because the
treewide ones hadn't been done for those files), I just took the SCSI
tree version as-is, even if it was old-style.  The old-style conversions
are perfectly valid, even if the "-only" and "-or-later" versions are
perhaps more descriptive.

a1b6fb947f scsi: libsas: no need to join wide port again in sas_ex_discover_dev()
Since we are processing events synchronously now, the second call of
sas_ex_join_wide_port() in sas_ex_discover_dev() is not needed. There will
be no races with other works in disco workqueue. So remove the second
sas_ex_join_wide_port().

I did not change the return value of 'res' to error when discover failed
because we need to continue to discover other phys if one phy discover
failed. So let's keep that logic as before and just add a debug log to
detect the failure. And directly return if second fanout expander attatched
to the parent expander because it has nothing to do after the phy is
disabled.

04cf8b32bc scsi: libsas: only clear phy->in_shutdown after shutdown event done
When the event queue is full of phy up and down events and reached the
threshold, we will queue a shutdown-event, and set phy->in_shutdown so that
we will not queue a shutdown-event again. But before the shutdown-event can
be executed, every phy-down event will clear
phy->in_shutdown and a new shutdown-event will be queued. The queue will
be full of these shutdown-events.

Fix this by only clear phy->in_shutdown in sas_phye_shutdown(), that is
after the first shutdown-event has been executed.

3c236f8cc6 scsi: libsas: Print expander PHY indexes in decimal
Currently we print expander PHY indexes in a mix of decimal and hex.

It is more consistent and also more convenient to read decimal, so
make this change.

We use width of 2 for expander and 1 for root PHYs prints.

Some lines which were needlessly spilling multiple lines are unified.

d8649fc1c5 scsi: libsas: Do discovery on empty PHY to update PHY info
When we discover the PHY is empty in sas_rediscover_dev(), the PHY
information (like negotiated linkrate) is not updated.

As such, for a user examining sysfs for that PHY, they would see
incorrect values:

root@(none)$ cd /sys/class/sas_phy/phy-0:0:20
root@(none)$ more negotiated_linkrate
3.0 Gbit
root@(none)$ echo 0 > enable
root@(none)$ more negotiated_linkrate
3.0 Gbit

So fix this, simply discover the PHY again, even though we know it's empty;
in the above example, this gives us:

root@(none)$ more negotiated_linkrate
Phy disabled

We must do this after unregistering the device associated with the PHY
(in sas_unregister_devs_sas_addr()).
