We have BIO_FLAG_LAST in the enum for bio specific flags, but it's
not used to check that we're not exceeding the size of them. Add
such a check.

This patch extends REQ_ALLOC_CACHE to IRQ completions, whenever
currently it's only limited to iopoll. Instead of guarding the list with
irq toggling on alloc, which is expensive, it keeps an additional
irq-safe list from which bios are spliced in batches to ammortise
overhead. On the put side it toggles irqs, but in many cases they're
already disabled and so cheap.

Biosets keep a mempool, so as long as requests complete we can always
can allocate and have forward progress. Percpu bio caches break that
assumptions as we may complete into the cache of one CPU and after try
and fail to allocate with another CPU. We also can't grab from another
CPU's cache without tricky sync.
If we're allocating with a bio while the mempool is undersaturated,
remove REQ_ALLOC_CACHE flag, so on put it will go straight to mempool.
It might try to free into mempool more requests than required, but
assuming than there is no memory starvation in the system it'll
stabilise and never hit that path.

When a bio's queue supports PCI P2PDMA, set FOLL_PCI_P2PDMA for
iov_iter_get_pages_flags(). This allows PCI P2PDMA pages to be passed
from userspace and enables the O_DIRECT path in iomap based filesystems
and direct to block devices.

Consecutive zone device pages should not be merged into the same sgl
or bvec segment with other types of pages or if they belong to different
pgmaps. Otherwise getting the pgmap of a given segment is not possible
without scanning the entire segment. This helper returns true either if
both pages are not zone device pages or both pages are zone device
pages with the same pgmap.
Add a helper to determine if zone device pages are mergeable and use
this helper in page_is_mergeable().

bio_put() with REQ_ALLOC_CACHE assumes that it's executed not from
an irq context. Let's add a warning if the invariant is not respected,
especially since there is a couple of places removing REQ_POLLED by hand
without also clearing REQ_ALLOC_CACHE.

KMSAN doesn't allow treating adjacent memory pages as such, if they were
allocated by different alloc_pages() calls. The block layer however does
so: adjacent pages end up being used together. To prevent this, make
page_is_mergeable() return false under KMSAN.

commit 9f5ede3c01 ("block: throttle split bio in case of iops limit")
support to count split bios for iops limit, thus it adds flagged bio
checking in tg_with_in_bps_limit() so that split bios will only count
once for bps limit, however, it introduce a new problem that io throttle
won't work if multiple bios are throttled.
In order to fix the problem, handle iops/bps limit in different ways:
1) for iops limit, there is no flag to record if the bio is throttled,
and iops is always applied.
2) for bps limit, original bio will be flagged with BIO_BPS_THROTTLED,
and io throttle will ignore bio with the flag.
Noted this patch also remove the code to set flag in __bio_clone(), it's
introduced in commit 111be88398 ("block-throttle: avoid double
charge"), and author thinks split bio can be resubmited and throttled
again, which is wrong because split bio will continue to dispatch from
caller.

This is useful for polled IO on a file, or for polled IO with the
io_uring passthrough mechanism. If bio allocations are done with
REQ_POLLED for those cases, then initializing the bio set with
BIOSET_PERCPU_CACHE enables the local per-cpu cache which eliminates
allocations (and frees) of bio structs when possible.

The size being added to a bio from an iov is aligned to a block size
after the pages were gotten. If the new aligned size truncates the last
page, its reference was being leaked. Ensure all pages that were not
added to the bio have their reference released.
Since this essentially requires doing the same that bio_put_pages(), and
there was only one caller for that function, this patch makes the
put_page() loop common for everyone.

Adding the page could fail on the bio_full() condition, which checks for
either exceeding the bio's max segments or total size exceeding
UINT_MAX. We already ensure the max segments can't be exceeded, so just
ensure the total size won't reach the limit. This simplifies error
handling and removes unnecessary repeated bio_full() checks.

There are cases where a bio may not accept additional pages, and the iov
needs to advance to the last data length that was accepted. The zone
append used to handle this correctly, but was inadvertently broken when
the setup was made common with the normal r/w case.

Allow using the splitting helpers on just a queue_limits instead of
a full request_queue structure. This will eventually allow file systems
or remapping drivers to split REQ_OP_ZONE_APPEND bios based on limits
calculated as the minimum common capabilities over multiple devices.

Use the address alignment requirements from the block_device for direct
io instead of requiring addresses be aligned to the block size. User
space can discover the alignment requirements from the dma_alignment
queue attribute.
User space can specify any hardware compatible DMA offset for each
segment, but every segment length is still required to be a multiple of
the block size.

Returning 0 early from __bio_iov_append_get_pages() for the
max_append_sectors warning just creates an infinite loop since 0 means
success, and the bio will never fill from the unadvancing iov_iter. We
could turn the return into an error value, but it will already be turned
into an error value later on, so just remove the warning. Clearly no one
ever hit it anyway.

Most of bioset_exit() is fine being called twice, as it clears the
various allocations etc when they are freed. The exception is
bio_alloc_cache_destroy(), which does not clear ->cache when it has
freed it.
This isn't necessarily a bug, but can be if buggy users does call the
exit path more then once, or with just a memset() bioset which has
never been initialized. dm appears to be one such user.

Appartly bcache can copy into bios that do not just contain fresh
pages but can have offsets into the bio_vecs. Restore support for tht

Remove the magic autofree semantics and require the callers to explicitly
call bio_init to initialize the bio.
This allows bio_free to catch accidental bio_put calls on bio_init()ed

Refine per-cpu bio alloc cache interfaces so that DM and other block
drivers can properly create and use the cache:
DM uses bioset_init_from_src() to do its final bioset initialization,
so must update bioset_init_from_src() to set BIOSET_PERCPU_CACHE if
%src bioset has a cache.
Also move bio_clear_polled() to include/linux/bio.h to allow users
outside of block core.

Replace the BIO_PERCPU_CACHE bio-internal flag with a REQ_ALLOC_CACHE
one that can be passed to bio_alloc / bio_alloc_bioset, and implement
the percpu cache allocation logic in a helper called from
bio_alloc_bioset. This allows any bio_alloc_bioset user to use the
percpu caches instead of having the functionality tied to struct kiocb.

a647a524a4 ("block: don't call rq_qos_ops->done_bio if the bio isn't
tracked") made bio_endio() skip rq_qos_done_bio() if BIO_TRACKED is not set.
While this fixed a potential oops, it also broke blk-iocost by skipping the
done_bio callback for merged bios.
Before, whether a bio goes through rq_qos_throttle() or rq_qos_merge(),
rq_qos_done_bio() would be called on the bio on completion with BIO_TRACKED
distinguishing the former from the latter. rq_qos_done_bio() is not called
for bios which wenth through rq_qos_merge(). This royally confuses
blk-iocost as the merged bios never finish and are considered perpetually
in-flight.
One reliably reproducible failure mode is an intermediate cgroup geting
stuck active preventing its children from being activated due to the
leaf-only rule, leading to loss of control. The following is from
resctl-bench protection scenario which emulates isolating a web server like
workload from a memory bomb run on an iocost configuration which should
yield a reasonable level of protection.

Fix it by introducing a new flag BIO_QOS_MERGED to mark merged bios and
calling rq_qos_done_bio() on them too. For consistency and clarity, rename
BIO_TRACKED to BIO_QOS_THROTTLED. The flag checks are moved into
rq_qos_done_bio() so that it's next to the code paths that set the flags.
With the patch applied, the above same benchmark shows:

__bio_clone_fast should also clone integrity and crypto data, as a clone
without those is incomplete. Right now the only caller that can actually
support crypto and integrity data (dm) does it manually for the one
callchain that supports these, but we better do it properly in the core.
Note that all callers except for the above mentioned one also don't need
to handle failure at all, given that the integrity and crypto clones are
based on mempool allocations that won't fail for sleeping allocations.

bio_truncate() clears the buffer outside of last block of bdev, however
current bio_truncate() is using the wrong offset of page. So it can
return the uninitialized data.
This happened when both of truncated/corrupted FS and userspace (via
bdev) are trying to read the last of bdev.

This is a thin wrapper around bio_add_page(). The main advantage here
is the documentation that folios larger than 2GiB are not supported.
It's not currently possible to allocate folios that large, but if it
ever becomes possible, this function will fail gracefully instead of
doing I/O to the wrong bytes.

Nobody cares about iov iterators state if we return -EIOCBQUEUED, so as
the we now have __blkdev_direct_IO_async(), which gets pages only once,
we can skip expensive iov_iter_advance(). It's around 1-2% of all CPU
spent.

Combine bio_iov_bvec_set() and bio_iov_bvec_set_append() and let the
caller to do iov_iter_advance(). Also get rid of __bio_iov_bvec_set(),
which was duplicated in the final binary, and replace a weird
iov_iter_truncate() of a temporal iter copy with min() better reflecting

Replace the blk_poll interface that requires the caller to keep a queue
and cookie from the submissions with polling based on the bio.
Polling for the bio itself leads to a few advantages:
- the cookie construction can made entirely private in blk-mq.c
- the caller does not need to remember the request_queue and cookie
separately and thus sidesteps their lifetime issues
- keeping the device and the cookie inside the bio allows to trivially
support polling BIOs remapping by stacking drivers
- a lot of code to propagate the cookie back up the submission path can
be removed entirely.

This flags ensures that the pages will not be reused for non-bio
allocations before the end of an RCU grace period. With that we can
safely use a RCU lookup for bio polling as long as we are fine with
occasionally polling the wrong device.

rq_qos framework is only applied on request based driver, so:
1) rq_qos_done_bio() needn't to be called for bio based driver
2) rq_qos_done_bio() needn't to be called for bio which isn't tracked,
such as bios ended from error handling code.
Especially in bio_endio():
1) request queue is referred via bio->bi_bdev->bd_disk->queue, which
may be gone since request queue refcount may not be held in above two
cases
2) q->rq_qos may be freed in blk_cleanup_queue() when calling into
__rq_qos_done_bio()
Fix the potential kernel panic by not calling rq_qos_ops->done_bio if
the bio isn't tracked. This way is safe because both ioc_rqos_done_bio()
and blkcg_iolatency_done_bio() are nop if the bio isn't tracked.

Add a per-cpu bio_set cache for bio allocations, enabling us to quickly
recycle them instead of going through the slab allocator. This cache
isn't IRQ safe, and hence is only really suitable for polled IO.
Very simple - keeps a count of bio's in the cache, and maintains a max
of 512 with a slack of 64. If we get above max + slack, we drop slack
number of bio's.

The memset() used is measurably slower in targeted benchmarks, wasting
about 1% of the total runtime, or 50% of the (later) hot path cached
bio alloc. Get rid of it and fill in the bio manually.

The function bio_trim has offset and size arguments that are declared
as int.
The callers of this function use sector_t type when passing the offset
and size, e.g. drivers/md/raid1.c:narrow_write_error() and
drivers/md/raid1.c:narrow_write_error().
Change offset and size arguments to sector_t type for bio_trim(). Also,
add WARN_ON_ONCE() to catch their overflow.

For chained bio, trace_block_bio_complete in bio_endio is currently called
only by the parent bio once upon all chained bio completed.
However, the sector and size for the parent bio are modified in bio_split.
Therefore, the size and sector of the complete events might not match the
queue events in blktrace.
The original fix of bio completion trace <fbbaf700e7b1> ("block: trace
completion of all bios.") wants multiple complete events to correspond
to one queue event but missed this.
The issue can be reproduced by md/raid5 read with bio cross chunks.
To fix, move trace completion into the loop for every chained bio to call.

bio size can grow up to 4GB when muli-page bvec is enabled.
but sometimes it would lead to inefficient behaviors.
in case of large chunk direct I/O, - 32MB chunk read in user space -
all pages for 32MB would be merged to a bio structure if the pages
physical addresses are contiguous. it makes some delay to submit
until merge complete. bio max size should be limited to a proper size.
When 32MB chunk read with direct I/O option is coming from userspace,
kernel behavior is below now in do_direct_IO() loop. it's timeline.

For multiple split bios, if one of the bio is fail, the whole
should return error to application. But we found there is a race
between bio_integrity_verify_fn and bio complete, which return
io success to application after one of the bio fail. The race as

The bio has been split as two: split and parent. When split
bio completed, it depends on kworker to do endio, while
bio_integrity_verify_fn have been interrupted by parent bio
complete irq handler. Then, parent bio->bi_status which have
been set in irq handler will overwrite by kworker.
In fact, even without the above race, we also need to conside
the concurrency beteen mulitple split bio complete and update
the same parent bi_status. Normally, multiple split bios will
be issued to the same hctx and complete from the same irq
vector. But if we have updated queue map between multiple split
bios, these bios may complete on different hw queue and different
irq vector. Then the concurrency update parent bi_status may
cause the final status error.

Christoph reported that we'll likely trigger the WARN_ON_ONCE() checking
that we're not submitting a bvec with REQ_OP_ZONE_APPEND in
bio_iov_iter_get_pages() some time ago using zoned btrfs, but I couldn't
reproduce it back then.
Now Naohiro was able to trigger the bug as well with xfstests generic/095
on a zoned btrfs.
There is nothing that prevents bvec submissions via REQ_OP_ZONE_APPEND if
the hardware's zone append limit is met.

Add bio_add_zone_append_page(), a wrapper around bio_add_hw_page() which
is intended to be used by file systems that directly add pages to a bio
instead of using bio_iov_iter_get_pages().

Instead of encoding of the bvec pool using magic bio flags, just use
a helper to find the pool based on the max_vecs value.

bio_iov_bvec_set clones the bio_vecs from the iter, and thus should be
treated like a cloned bio in every respect. That also includes not
touching bi_max_vecs as that is a property of the bio allocation and not
its current payload.

bio_iov_bvec_set assigns the foreign bvec, so setting the NO_PAGE_REF
directly there seems like the best fit.

The bi_max_vecs and bi_vcnt fields are defined as unsigned short, so
don't allow passing larger values in.

All bios with up to 4 bvecs use the inline bvecs in the bio itself, so
don't bother to define bvec_slabs entries for them. Also decruftify
the bvec_slabs definition and initialization while we're at it.
