With _F_DESC_ASID backend feature, the device can now support the
VHOST_VDPA_GET_VRING_DESC_GROUP ioctl, and it may expose the descriptor
table (including avail and used ring) in a different group than the
buffers it contains. This new uAPI will fetch the group ID of the
descriptor table.

Userspace knows if the device has dedicated descriptor group or not
by checking this feature bit.
It's only exposed if the vdpa driver backend implements the
.get_vq_desc_group() operation callback. Userspace trying to negotiate
this feature when it or the dependent _F_IOTLB_ASID feature hasn't
been exposed will result in an error.

Commit e2ae38cf3d ("vhost: fix hung thread due to erroneous iotlb
entries") Forbade vhost iotlb msg with null size to prevent entries
with size = start = 0 and last = ULONG_MAX to end up in the iotlb.
Then commit 95932ab2ea ("vhost: allow batching hint without size")
only applied the check for VHOST_IOTLB_UPDATE and VHOST_IOTLB_INVALIDATE
message types to fix a regression observed with batching hit.
Still, the introduction of that check introduced a regression for
some users attempting to invalidate the whole ULONG_MAX range by
setting the size to 0. This is the case with qemu/smmuv3/vhost
integration which does not work anymore. It Looks safe to partially
revert the original commit and allow VHOST_IOTLB_INVALIDATE messages
with null size. vhost_iotlb_del_range() will compute a correct end
iova. Same for vhost_vdpa_iotlb_unmap().

This allows userspace to request the fabric drivers do direct submissions
if they support it. With the new device file, submit_type, users can
write 0 - 2 to control how commands are submitted to the backend:
0 - TARGET_FABRIC_DEFAULT_SUBMIT - LIO will use the fabric's default
submission type. This is the default for compat.
1 - TARGET_DIRECT_SUBMIT - LIO will submit the cmd to the backend from the
calling context if the fabric the cmd was received on supports it,
else it will use the fabric's default type.
2 - TARGET_QUEUE_SUBMIT - LIO will queue the cmd to the LIO submission
workqueue which will pass it to the backend.
When using an NVMe drive and vhost-scsi with direct submission we see
around a 20% improvement in 4K I/Os:

And when using the queueing mode, we now no longer see issues like where

In some cases, like with multiple LUN targets or where the target has to
respond to transport level requests from the receiving context it can be
better to defer cmd submission to a helper thread. If the backend driver
blocks on something like request/tag allocation it can block the entire
target submission path and other LUs and transport IO on that session.
In other cases like single LUN targets with storage that can support all
the commands that the target can queue, then it's best to submit the cmd
to the backend from the target's cmd receiving context.
Subsequent commits will allow the user to config what they prefer, but
drivers like loop can't directly submit because they can be called from a
context that can't sleep. And, drivers like vhost-scsi can support direct
submission, but need to keep their default behavior of deferring execution
to avoid possible regressions where the backend can block.
Make the drivers tell LIO core if they support direct submissions and their
current default, so we can prevent users from misconfiguring the system and
initialize devices correctly.

In the while loop of vringh_iov_xfer(), `partlen` could be 0 if one of
the `iov` has 0 lenght.
In this case, we should skip the iov and go to the next one.
But calling vringh_kiov_advance() with 0 lenght does not cause the
advancement, since it returns immediately if asked to advance by 0 bytes.
Let's restore the code that was there before commit b8c06ad4d6
("vringh: implement vringh_kiov_advance()"), avoiding using
vringh_kiov_advance().

This is preparation patch for MSG_ZEROCOPY support. It adds handling of
non-linear skbs by replacing direct calls of 'memcpy_to_msg()' with
'skb_copy_datagram_iter()'. Main advantage of the second one is that it
can handle paged part of the skb by using 'kmap()' on each page, but if
there are no pages in the skb, it behaves like simple copying to iov
iterator. This patch also adds new field to the control block of skb -
this value shows current offset in the skb to read next portion of data
(it doesn't matter linear it or not). Idea behind this field is that
'skb_copy_datagram_iter()' handles both types of skb internally - it
just needs an offset from which to copy data from the given skb. This
offset is incremented on each read from skb. This approach allows to
simplify handling of both linear and non-linear skbs, because for
linear skb we need to call 'skb_pull()' after reading data from it,
while in non-linear case we need to update 'data_len'.

The linux block layer requires bios/requests to have lengths with a 512
byte alignment. Some drivers/layers like dm-crypt and the directi IO code
will test for it and just fail. Other drivers like SCSI just assume the
requirement is met and will end up in infinte retry loops. The problem
for drivers like SCSI is that it uses functions like blk_rq_cur_sectors
and blk_rq_sectors which divide the request's length by 512. If there's
lefovers then it just gets dropped. But other code in the block/scsi
layer may use blk_rq_bytes/blk_rq_cur_bytes and end up thinking there is
still data left and try to retry the cmd. We can then end up getting
stuck in retry loops where part of the block/scsi thinks there is data
left, but other parts think we want to do IOs of zero length.
Linux will always check for alignment, but windows will not. When
vhost-scsi then translates the iovec it gets from a windows guest to a
scatterlist, we can end up with sg items where the sg->length is not
divisible by 512 due to the misaligned offset:

sg[N].length = 255;
When the lio backends then convert the SG to bios or other iovecs, we
end up sending them with the same misaligned values and can hit the
issues above.
This just has us drop down to allocating a temp page and copying the data
when we detect a misaligned buffer and the IO is large enough that it
will get split into multiple bad IOs.

This patch drops the requirement that we can only switch workers if work
has not been queued by using RCU for the vq based queueing paths and a
mutex for the device wide flush.
We can also use this to support SIGKILL properly in the future where we
should exit almost immediately after getting that signal. With this
patch, when get_signal returns true, we can set the vq->worker to NULL
and do a synchronize_rcu to prevent new work from being queued to the
vhost_task that has been killed.

This has vhost-scsi support the worker ioctls by calling the
vhost_worker_ioctl helper.
With a single worker, the single thread becomes a bottlneck when trying
to use 3 or more virtqueues like:
fio --filename=/dev/sdb --direct=1 --rw=randrw --bs=4k \
--ioengine=libaio --iodepth=128 --numjobs=3
With the patches and doing a worker per vq, we can scale to at least
16 vCPUs/vqs (that's my system limit) with the same command fio command
above with numjobs=16:
fio --filename=/dev/sdb --direct=1 --rw=randrw --bs=4k \
--ioengine=libaio --iodepth=64 --numjobs=16
which gives around 2002K IOPs.
Note that for testing I dropped depth to 64 above because the vhost/virt
layer supports only 1024 total commands per device. And the only tuning I
did was set LIO's emulate_pr to 0 to avoid LIO's PR lock in the main IO
path which becomes an issue at around 12 jobs/virtqueues.

For vhost-scsi with 3 vqs or more and a workload that tries to use
them in parallel like:
fio --filename=/dev/sdb --direct=1 --rw=randrw --bs=4k \
--ioengine=libaio --iodepth=128 --numjobs=3
the single vhost worker thread will become a bottlneck and we are stuck
at around 500K IOPs no matter how many jobs, virtqueues, and CPUs are
used.
To better utilize virtqueues and available CPUs, this patch allows
userspace to create workers and bind them to vqs. You can have N workers
per dev and also share N workers with M vqs on that dev.
This patch adds the interface related code and the next patch will hook
vhost-scsi into it. The patches do not try to hook net and vsock into
the interface because:
1. multiple workers don't seem to help vsock. The problem is that with
only 2 virtqueues we never fully use the existing worker when doing
bidirectional tests. This seems to match vhost-scsi where we don't see
the worker as a bottleneck until 3 virtqueues are used.
2. net already has a way to use multiple workers.

With one worker we will always send the scsi cmd responses then send the
TMF rsp, because LIO will always complete the scsi cmds first then call
into us to send the TMF response.
With multiple workers, the IO vq workers could be running while the
TMF/ctl vq worker is running so this has us do a flush before completing
the TMF to make sure cmds are completed when it's work is later queued
and run.

This patch separates the scsi cmd completion code paths so we can complete
cmds based on their vq instead of having all cmds complete on the same
worker/CPU. This will be useful with the next patches that allow us to
create mulitple worker threads and bind them to different vqs, and we can
have completions running on different threads/CPUs.

This has the drivers pass in their poll to vq mapping and then converts
the core poll code to use the vq based helpers. In the next patches we
will allow vqs to be handled by different workers, so to allow drivers
to execute operations like queue, stop, flush, etc on specific polls/vqs
we need to know the mappings.

support multiple workers we can then flush each worker during device
removal, stoppage, etc. It also adds a helper to flush specific

support multiple workers. It also adds a helper that takes a vq during
queueing so modules can control which vq/worker to queue work on.

vsock can start queueing work after VHOST_VSOCK_SET_GUEST_CID, so
after we have called vhost_worker_create it can be calling
vhost_work_queue and trying to access the vhost worker/task. If
vhost_dev_alloc_iovecs fails, then vhost_worker_free could free
the worker/task from under vsock.
This moves vhost_worker_create to the end of vhost_dev_set_owner
where we know we can no longer fail in that path. If it fails
after the VHOST_SET_OWNER and userspace closes the device, then
the normal vsock release handling will do the right thing.

We are now in a position where no caller of pin_user_pages() requires the
vmas parameter at all, so eliminate this parameter from the function and
all callers.
This clears the way to removing the vmas parameter from GUP altogether.

We can race where we have added work to the work_list, but
vhost_task_fn has passed that check but not yet set us into
TASK_INTERRUPTIBLE. wake_up_process will see us in TASK_RUNNING and
just return.
This bug was intoduced in commit f9010dbdce ("fork, vhost: Use
CLONE_THREAD to fix freezer/ps regression") when I moved the setting
of TASK_INTERRUPTIBLE to simplfy the code and avoid get_signal from
logging warnings about being in the wrong state. This moves the setting
of TASK_INTERRUPTIBLE back to before we test if we need to stop the
task to avoid a possible race there as well. We then have vhost_worker
set TASK_RUNNING if it finds work similar to before.

If userspace does VHOST_VSOCK_SET_GUEST_CID before VHOST_SET_OWNER we
can race where:
1. thread0 calls vhost_transport_send_pkt -> vhost_work_queue
2. thread1 does VHOST_SET_OWNER which calls vhost_worker_create.
3. vhost_worker_create will set the dev->worker pointer before setting
the worker->vtsk pointer.
4. thread0's vhost_work_queue will see the dev->worker pointer is
set and try to call vhost_task_wake using not yet set worker->vtsk
pointer.
5. We then crash since vtsk is NULL.
Before commit 6e890c5d50 ("vhost: use vhost_tasks for worker
threads"), we only had the worker pointer so we could just check it to
see if VHOST_SET_OWNER has been done. After that commit we have the
vhost_worker and vhost_task pointer, so we can now hit the bug above.
This patch embeds the vhost_worker in the vhost_dev and moves the work
list initialization back to vhost_dev_init, so we can just check the
worker.vtsk pointer to check if VHOST_SET_OWNER has been done like
before.

Fix possible virtqueue used buffers leak and corresponding stuck
in case of temporary -EIO from sendmsg() which is produced by
tun driver while backend device is not up.
In case of no-retriable error and zcopy do not revert upend_idx
to pass packet data (that is update used_idx in corresponding
vhost_zerocopy_signal_used()) as if packet data has been
transferred successfully.

When switching from kthreads to vhost_tasks two bugs were added:
1. The vhost worker tasks's now show up as processes so scripts doing
ps or ps a would not incorrectly detect the vhost task as another
process. 2. kthreads disabled freeze by setting PF_NOFREEZE, but
vhost tasks's didn't disable or add support for them.
To fix both bugs, this switches the vhost task to be thread in the
process that does the VHOST_SET_OWNER ioctl, and has vhost_worker call
get_signal to support SIGKILL/SIGSTOP and freeze signals. Note that
SIGKILL/STOP support is required because CLONE_THREAD requires
CLONE_SIGHAND which requires those 2 signals to be supported.
This is a modified version of the patch written by Mike Christie
<michael.christie@oracle.com> which was a modified version of patch
originally written by Linus.
Much of what depended upon PF_IO_WORKER now depends on PF_USER_WORKER.
Including ignoring signals, setting up the register state, and having
get_signal return instead of calling do_group_exit.
Tidied up the vhost_task abstraction so that the definition of
vhost_task only needs to be visible inside of vhost_task.c. Making
it easier to review the code and tell what needs to be done where.
As part of this the main loop has been moved from vhost_worker into
vhost_task_fn. vhost_worker now returns true if work was done.
The main loop has been updated to call get_signal which handles
SIGSTOP, freezing, and collects the message that tells the thread to
exit as part of process exit. This collection clears
__fatal_signal_pending. This collection is not guaranteed to
clear signal_pending() so clear that explicitly so the schedule()
sleeps.
For now the vhost thread continues to exist and run work until the
last file descriptor is closed and the release function is called as
part of freeing struct file. To avoid hangs in the coredump
rendezvous and when killing threads in a multi-threaded exec. The
coredump code and de_thread have been modified to ignore vhost threads.
Remvoing the special case for exec appears to require teaching
vhost_dev_flush how to directly complete transactions in case
the vhost thread is no longer running.
Removing the special case for coredump rendezvous requires either the
above fix needed for exec or moving the coredump rendezvous into
get_signal.

the driver core in the quest to be able to move "struct bus" and "struct
class" into read-only memory, a task now complete with these changes.
This will make the future rust interactions with the driver core more
"provably correct" as well as providing more obvious lifetime rules for
all busses and classes in the kernel.
The changes required for this did touch many individual classes and
busses as many callbacks were changed to take const * parameters
instead. All of these changes have been submitted to the various
subsystem maintainers, giving them plenty of time to review, and most of
them actually did so.

things:

"This contains the work generalizing the ability to create a kernel
worker from a userspace process.
Such user workers will run with the same credentials as the userspace
process they were created from providing stronger security and
accounting guarantees than the traditional override_creds() approach
ever could've hoped for.
The original work was heavily based and optimzed for the needs of
io_uring which was the first user. However, as it quickly turned out
the ability to create user workers inherting properties from a
userspace process is generally useful.
The vhost subsystem currently creates workers using the kthread api.
The consequences of using the kthread api are that RLIMITs don't work
correctly as they are inherited from khtreadd. This leads to bugs
where more workers are created than would be allowed by the RLIMITs of
the userspace process in lieu of which workers are created.
Problems like this disappear with user workers created from the
userspace processes for which they perform the work. In addition,
providing this api allows vhost to remove additional complexity. For
example, cgroup and mm sharing will just work out of the box with user
workers based on the relevant userspace process instead of manually
ensuring the correct cgroup and mm contexts are used.
So the vhost subsystem should simply be made to use the same mechanism
as io_uring. To this end the original mechanism used for
create_io_thread() is generalized into user workers:
- Introduce PF_USER_WORKER as a generic indicator that a given task
is a user worker, i.e., a kernel task that was created from a
userspace process. Now a PF_IO_WORKER thread is just a specialized
version of PF_USER_WORKER. So io_uring io workers raise both flags.
- Make copy_process() available to core kernel code
- Extend struct kernel_clone_args with the following bitfields
allowing to indicate to copy_process():
- to create a user worker (raise PF_USER_WORKER)
- to not inherit any files from the userspace process
- to ignore signals
After all generic changes are in place the vhost subsystem implements
a new dedicated vhost api based on user workers. Finally, vhost is
switched to rely on the new api moving it off of kthreads.

While using the vdpa device with vIOMMU enabled
in the guest VM, when the vdpa device bind to vfio-pci and run testpmd
then system will fail to unmap.
The test process is
Load guest VM --> attach to virtio driver--> bind to vfio-pci driver
So the mapping process is
1)batched mode map to normal MR
2)batched mode unmapped the normal MR
3)unmapped all the memory
4)mapped to iommu MR
This error happened in step 3). The iotlb was freed in step 2)
and the function vhost_vdpa_process_iotlb_msg will return fail
Which causes failure.
To fix this, we will not remove the AS while the iotlb->nmaps is 0.
This will free in the vhost_vdpa_clean

kmap_atomic() is deprecated in favor of kmap_local_page() since commit
f3ba3c710a ("mm/highmem: Provide kmap_local*").
With kmap_local_page() the mappings are per thread, CPU local, can take
page-faults, and can be called from any context (including interrupts).
Furthermore, the tasks can be preempted and, when they are scheduled to
run again, the kernel virtual addresses are restored and still valid.
kmap_atomic() is implemented like a kmap_local_page() which also disables
page-faults and preemption (the latter only for !PREEMPT_RT kernels,
otherwise it only disables migration).
The code within the mappings/un-mappings in getu16_iotlb() and
putu16_iotlb() don't depend on the above-mentioned side effects of
kmap_atomic(), so that mere replacements of the old API with the new one
is all that is required (i.e., there is no need to explicitly add calls
to pagefault_disable() and/or preempt_disable()).
This commit reuses a "boiler plate" commit message from Fabio, who has
already did this change in several places.

When the user call VHOST_SET_OWNER ioctl and the vDPA device
has `use_va` set to true, let's call the bind_mm callback.
In this way we can bind the device to the user address space
and directly use the user VA.
The unbind_mm callback is called during the release after
stopping the device.

We on longer need to hold the vhost_scsi_mutex the entire time we
set/clear the endpoint. The tv_tpg_mutex handles tpg accesses not related
to the tpg list, the port link/unlink functions use the tv_tpg_mutex while
accessing the tpg->vhost_scsi pointer, vhost_scsi_do_plug will no longer
queue events after the virtqueue's backend has been cleared and flushed,
and we don't drop our refcount to the tpg until after we have stopped
cmds and wait for outstanding cmds to complete.
This moves the vhost_scsi_mutex use to it's documented use of being used
to access the tpg list. We then don't need to hold it while a flush is
being performed causing other device's vhost_scsi_set_endpoint
and vhost_scsi_make_tpg/vhost_scsi_drop_tpg calls to have to wait on a
flakey device.

We are using the vhost_scsi_mutex to make sure vhost_scsi_port_link and
vhost_scsi_port_unlink see if vhost_scsi_clear_endpoint has cleared
tpg->vhost_scsi and it can't be freed while they are using.
However, we currently set the tpg->vhost_scsi pointer while holding
tv_tpg_mutex. So, we can just hold that while calling
vhost_scsi_hotplug/hotunplug. We then don't need to hold the
vhost_scsi_mutex while vhost_scsi_clear_endpoint is holding it and doing
a flush which could cause the LUN map/unmap to have to wait on another
device's flush.

We currenly hold the vhost_scsi_mutex while clearing the endpoint and
while performing vhost_scsi_do_plug, so tpg->vhost_scsi can't be freed
from uder us, and to make sure anything queued is handled by the
full call in vhost_scsi_clear_endpoint.
This patch removes the need for the vhost_scsi_mutex for the latter
case. In the next patches, we won't hold the vhost_scsi_mutex while
flushing so this patch adds a check for the clearing of the virtqueue
from vhost_scsi_clear_endpoint. We then know that once
vhost_scsi_clear_endpoint has cleared the backend that no new events
will be queued, and the flush after the vhost_vq_set_backend(vq, NULL)
call will see everything that's been queued to that point. So the flush
will then handle all events without the need for the vhost_scsi_mutex.

We don't need the device mutex in vhost_scsi_do_plug because:
1. we have the vhost_scsi_mutex so the tpg->vhost_scsi pointer will not
change on us and the vhost_scsi can't be freed from under us if it was
set.
2. vhost_scsi_clear_endpoint will stop the virtqueues and flush them while
holding the vhost_scsi_mutex so we know once vhost_scsi_clear_endpoint
has completed that vhost_scsi_do_plug can't send new events and any
queued ones have completed.
So this patch drops the device mutex use in vhost_scsi_do_plug.

We currently hold the vhost_scsi_mutex the entire time we are running
vhost_scsi_clear_endpoint. One of the reasons for this is that it prevents
userspace from being able to free the se_tpg from under us after we have
called target_undepend_item. However, it forces management operations for
for other devices to have to wait on a flakey device's:
vhost_scsi_clear_endpoint -> vhost_scsi_flush()
call which can which can take a long time.
This moves the target_undepend_item call and the tpg unsetup code to after
we have stopped new IO from starting up and after we have waited on
running IO. We can then release our refcount on the tpg and session
knowing our device is no longer accessing them. We can then drop the
vhost_scsi_mutex use during thee flush call in later patches in this set,
when we have removed other reasons for holding it.

The vhost_get_avail_size and vhost_get_used_size functions compute the size
of structures with flexible array members with an additional 2 bytes if the
VIRTIO_RING_F_EVENT_IDX feature flag is set. Convert these functions to use
struct_size() and size_add() instead of coding the calculation by hand.
This ensures that the calculations will saturate at SIZE_MAX rather than
overflowing.

We normally clear the endpoint then unmap LUNs so the devices are fully
shutdown when the LUN is unmapped, but it's legal to unmap before
clearing. If the user does that while TMFs are running then we can end
up crashing.
vhost_scsi_port_unlink assumes that the LUN's tmf struct will always be on
the tmf_queue list. However, if a TMF is running then it will have been
removed while it's executing. If we do a LUN unmap at this time, then
we assume the entry is on the list and just start accessing it and free
it.
This fixes the bug by just allocating the vhost_scsi_tmf struct when it's
needed like is done with the se_tmr struct that's needed when we submit
the TMF. In this path perf is not an issue and we can use GFP_KERNEL
since it won't swing directly back on us, so we don't need to preallocate
the struct.

If vhost_scsi_setup_vq_cmds fails we leave the tpg->vhost_scsi pointer
set. If the device is freed and then the user unmaps the LUN, the call to
vhost_scsi_port_unlink -> vhost_scsi_hotunplug will see the that
tpg->vhost_scsi is still set and try to use it.
This has us clear the vhost_scsi pointer in the failure path. It also
has us take tv_tpg_mutex in this failure path, because tv_tpg_vhost_count
is accessed under this mutex in vhost_scsi_drop_nexus and in the future
we will want to serialize access to tpg->vhost_scsi with that mutex
instead of the vhost_scsi_mutex.

This returns a pointer to the current iovec entry in the iterator. Only
useful with ITER_IOVEC right now, but it prepares us to treat ITER_UBUF
and ITER_IOVEC identically for the first segment.
Rename struct iov_iter->iov to iov_iter->__iov to find any potentially
troublesome spots, and also to prevent anyone from adding new code that
accesses iter->iov directly.

For vhost workers we use the kthread API which inherit's its values from
and checks against the kthreadd thread. This results in the wrong RLIMITs
being checked, so while tools like libvirt try to control the number of
threads based on the nproc rlimit setting we can end up creating more
threads than the user wanted.
This patch has us use the vhost_task helpers which will inherit its
values/checks from the thread that owns the device similar to if we did
a clone in userspace. The vhost threads will now be counted in the nproc
rlimits. And we get features like cgroups and mm sharing automatically,
so we can remove those calls.

Qemu will create vhost devices in the kernel which perform network, SCSI,
etc IO and management operations from worker threads created by the
kthread API. Because the kthread API does a copy_process on the kthreadd
thread, the vhost layer has to use kthread_use_mm to access the Qemu
thread's memory and cgroup_attach_task_all to add itself to the Qemu
thread's cgroups, and it bypasses the RLIMIT_NPROC limit which can result
in VMs creating more threads than the admin expected.
This patch adds a new struct vhost_task which can be used instead of
kthreads. They allow the vhost layer to use copy_process and inherit
the userspace process's mm and cgroups, the task is accounted for
under the userspace's nproc count and can be seen in its process tree,
and other features like namespaces work and are inherited by default.

Livepatch timeouts were reported due to busy vhost_worker() kthreads.
Now that cond_resched() can do livepatch task switching, use
cond_resched() in vhost_worker(). That's the better way to
conditionally call schedule() anyway.

Currently vhost_vdpa_cleanup() unmaps the DMA mappings by calling
`iommu_unmap(v->domain, map->start, map->size);`
from vhost_vdpa_general_unmap() when the parent vDPA driver doesn't
provide DMA config operations.
However, the IOMMU domain referred to by `v->domain` is freed in
vhost_vdpa_free_domain() before vhost_vdpa_cleanup() in
vhost_vdpa_release() which results in NULL pointer de-reference.
Accordingly, moving the call to vhost_vdpa_free_domain() in
vhost_vdpa_cleanup() would makes sense. This will also help
detaching the dma device in error handling of vhost_vdpa_alloc_domain().
This issue was observed on terminating QEMU with SIGQUIT.

Add VIRTIO_F_RING_RESET, which indicates that the driver can reset a
queue individually.
VIRTIO_F_RING_RESET feature is added to virtio-spec 1.2. The relevant
information is in
oasis-tcs/virtio-spec#124oasis-tcs/virtio-spec#139
The implementation only adds the feature bit in supported features. It
does not require any other changes because we reuse the existing vhost
protocol.
The virtqueue reset process can be concluded as two parts:
1. The driver can reset a virtqueue. When it is triggered, we use the
set_backend to disable the virtqueue.
2. After the virtqueue is disabled, the driver may optionally re-enable
it. The process is basically similar to when the device is started,
except that the restart process does not need to set features and set
mem table since they do not change. QEMU will send messages containing
size, base, addr, kickfd and callfd of the virtqueue in order.
Specifically, the host kernel will receive these messages in order:
a. VHOST_SET_VRING_NUM
b. VHOST_SET_VRING_BASE
c. VHOST_SET_VRING_ADDR
d. VHOST_SET_VRING_KICK
e. VHOST_SET_VRING_CALL
f. VHOST_NET_SET_BACKEND
Finally, after we use set_backend to attach the virtqueue, the virtqueue
will be enabled and start to work.

This new ioctl adds support for resuming the device from userspace.
This is required when trying to restore the device in a functioning
state after it's been suspended. It is already possible to reset a
suspended device, but that means the device must be reconfigured and
all the IOMMU/IOTLB mappings must be recreated. This new operation
allows the device to be resumed without going through a full reset.
This is particularly useful when trying to perform offline migration of
a virtual machine (also known as snapshot/restore) as it allows the VMM
to resume the virtual machine back to a running state after the snapshot
is performed.

Userspace knows if the device can be resumed or not by checking this
feature bit.
It's only exposed if the vdpa driver backend implements the resume()
operation callback. Userspace trying to negotiate this feature when it
hasn't been exposed will result in an error.

However, the spec doesn't say anything about the legacy descriptor
layout for the respone. So this patch tries to not assume the response
to reside in a single separate descriptor which is what commit
79c14141a4 ("vhost/scsi: Convert completion path to use") tries to
achieve towards to ANY_LAYOUT.
This is done by allocating and using dedicate resp iov in the
command. To be safety, start with UIO_MAXIOV to be consistent with the
limitation that we advertise to the vhost_get_vq_desc().
Testing with the hacked virtio-scsi driver that use 1 descriptor for 1
byte in the response.

When the vhost iotlb is used along with a guest virtual iommu
and the guest gets rebooted, some MISS messages may have been
recorded just before the reboot and spuriously executed by
the virtual iommu after the reboot.
As vhost does not have any explicit reset user API,
VHOST_NET_SET_BACKEND looks a reasonable point where to clear
the pending messages, in case the backend is removed.
Export vhost_clear_msg() and call it in vhost_net_set_backend()
when fd == -1.

The internal mechanisms support this, but instead of exposting the gfp to
the caller it wrappers it into iommu_map() and iommu_map_atomic()
Fix this instead of adding more variants for GFP_KERNEL_ACCOUNT.

This commit changes virtio/vsock to use sk_buff instead of
virtio_vsock_pkt. Beyond better conforming to other net code, using
sk_buff allows vsock to use sk_buff-dependent features in the future
(such as sockmap) and improves throughput.
