0a0f0d8be7 dma-mapping: split <linux/dma-mapping.h>
Split out all the bits that are purely for dma_map_ops implementations
and related code into a new <linux/dma-map-ops.h> header so that they
don't get pulled into all the drivers.  That also means the architecture
specific <asm/dma-mapping.h> is not pulled in by <linux/dma-mapping.h>
any more, which leads to a missing includes that were pulled in by the
x86 or arm versions in a few not overly portable drivers.

4af22ded0e arc: fix memory initialization for systems with two memory banks
Rework of memory map initialization broke initialization of ARC systems
with two memory banks. Before these changes, memblock was not aware of
nodes configuration and the memory map was always allocated from the
"lowmem" bank. After the addition of node information to memblock, the core
mm attempts to allocate the memory map for the "highmem" bank from its
node. The access to this memory using __va() fails because it can be only
accessed using kmap.

Anther problem that was uncovered is that {min,max}_high_pfn are calculated
from u64 high_mem_start variable which prevents truncation to 32-bit
physical address and the PFN values are above the node and zone boundaries.

Use phys_addr_t type for high_mem_start and high_mem_size to ensure
correspondence between PFNs and highmem zone boundaries and reserve the
entire highmem bank until mem_init() to avoid accesses to it before highmem
is enabled.

To test this:
1. Enable HIGHMEM in ARC config
2. Enable 2 memory banks in haps_hs.dts (uncomment the 2nd bank)

52e3f8d030 mm/arc: use general page fault accounting
Use the general page fault accounting by passing regs into
handle_mm_fault().  It naturally solve the issue of multiple page fault
accounting when page fault retry happened.

Fix PERF_COUNT_SW_PAGE_FAULTS perf event manually for page fault retries,
by moving it before taking mmap_sem.

bce617edec mm: do page fault accounting in handle_mm_fault
Patch series "mm: Page fault accounting cleanups", v5.

This is v5 of the pf accounting cleanup series.  It originates from Gerald
Schaefer's report on an issue a week ago regarding to incorrect page fault
accountings for retried page fault after commit 4064b98270 ("mm: allow
VM_FAULT_RETRY for multiple times"):

What this series did:

  - Correct page fault accounting: we do accounting for a page fault
    (no matter whether it's from #PF handling, or gup, or anything else)
    only with the one that completed the fault.  For example, page fault
    retries should not be counted in page fault counters.  Same to the
    perf events.

  - Unify definition of PERF_COUNT_SW_PAGE_FAULTS: currently this perf
    event is used in an adhoc way across different archs.

    Case (1): for many archs it's done at the entry of a page fault
    handler, so that it will also cover e.g.  errornous faults.

    Case (2): for some other archs, it is only accounted when the page
    fault is resolved successfully.

    Case (3): there're still quite some archs that have not enabled
    this perf event.

    Since this series will touch merely all the archs, we unify this
    perf event to always follow case (1), which is the one that makes most
    sense.  And since we moved the accounting into handle_mm_fault, the
    other two MAJ/MIN perf events are well taken care of naturally.

  - Unify definition of "major faults": the definition of "major
    fault" is slightly changed when used in accounting (not
    VM_FAULT_MAJOR).  More information in patch 1.

  - Always account the page fault onto the one that triggered the page
    fault.  This does not matter much for #PF handlings, but mostly for
    gup.  More information on this in patch 25.

Patchset layout:

Patch 1:     Introduced the accounting in handle_mm_fault(), not enabled.
Patch 2-23:  Enable the new accounting for arch #PF handlers one by one.
Patch 24:    Enable the new accounting for the rest outliers (gup, iommu, etc.)
Patch 25:    Cleanup GUP task_struct pointer since it's not needed any more

This patch (of 25):

This is a preparation patch to move page fault accountings into the
general code in handle_mm_fault().  This includes both the per task
flt_maj/flt_min counters, and the major/minor page fault perf events.  To
do this, the pt_regs pointer is passed into handle_mm_fault().

PERF_COUNT_SW_PAGE_FAULTS should still be kept in per-arch page fault
handlers.

So far, all the pt_regs pointer that passed into handle_mm_fault() is
NULL, which means this patch should have no intented functional change.

ca15ca406f mm: remove unneeded includes of <asm/pgalloc.h>
Patch series "mm: cleanup usage of <asm/pgalloc.h>"

Most architectures have very similar versions of pXd_alloc_one() and
pXd_free_one() for intermediate levels of page table.  These patches add
generic versions of these functions in <asm-generic/pgalloc.h> and enable
use of the generic functions where appropriate.

In addition, functions declared and defined in <asm/pgalloc.h> headers are
used mostly by core mm and early mm initialization in arch and there is no
actual reason to have the <asm/pgalloc.h> included all over the place.
The first patch in this series removes unneeded includes of
<asm/pgalloc.h>

In the end it didn't work out as neatly as I hoped and moving
pXd_alloc_track() definitions to <asm-generic/pgalloc.h> would require
unnecessary changes to arches that have custom page table allocations, so
I've decided to move lib/ioremap.c to mm/ and make pgalloc-track.h local
to mm/.

This patch (of 8):

In most cases <asm/pgalloc.h> header is required only for allocations of
page table memory.  Most of the .c files that include that header do not
use symbols declared in <asm/pgalloc.h> and do not require that header.

As for the other header files that used to include <asm/pgalloc.h>, it is
possible to move that include into the .c file that actually uses symbols
from <asm/pgalloc.h> and drop the include from the header file.

The process was somewhat automated using

where /tmp/xx contains all the symbols defined in

20b271dfe9 arch/kmap: define kmap_atomic_prot() for all arch's
To support kmap_atomic_prot(), all architectures need to support
protections passed to their kmap_atomic_high() function.  Pass protections
into kmap_atomic_high() and change the name to kmap_atomic_high_prot() to
match.

Then define kmap_atomic_prot() as a core function which calls
kmap_atomic_high_prot() when needed.

Finally, redefine kmap_atomic() as a wrapper of kmap_atomic_prot() with
the default kmap_prot exported by the architectures.

525aaf9bad arch/kmap: remove redundant arch specific kmaps
The kmap code for all the architectures is almost 100% identical.

Lift the common code to the core.  Use ARCH_HAS_KMAP_FLUSH_TLB to indicate
if an arch defines kmap_flush_tlb() and call if if needed.

This also has the benefit of changing kmap() on a number of architectures
to be an inline call rather than an actual function.

01c4b788e0 arch/kmap: remove BUG_ON()
Patch series "Remove duplicated kmap code", v3.

The kmap infrastructure has been copied almost verbatim to every
architecture.  This series consolidates obvious duplicated code by
defining core functions which call into the architectures only when
needed.

Some of the k[un]map_atomic() implementations have some similarities but
the similarities were not sufficient to warrant further changes.

In addition we remove a duplicate implementation of kmap() in DRM.

This patch (of 15):

Replace the use of BUG_ON(in_interrupt()) in the kmap() and kunmap() in
favor of might_sleep().

Besides the benefits of might_sleep(), this normalizes the implementations
such that they can be made generic in subsequent patches.

Some architectures (e.g.  ARC) have the ZONE_HIGHMEM zone below the
ZONE_NORMAL.  Allowing free_area_init() parse max_zone_pfn array even it
is sorted in descending order allows using free_area_init() on such
architectures.

Add top -> down traversal of max_zone_pfn array in free_area_init() and
use the latter in ARC node/zone initialization.

4064b98270 mm: allow VM_FAULT_RETRY for multiple times
The idea comes from a discussion between Linus and Andrea [1].

Before this patch we only allow a page fault to retry once.  We achieved
this by clearing the FAULT_FLAG_ALLOW_RETRY flag when doing
handle_mm_fault() the second time.  This was majorly used to avoid
unexpected starvation of the system by looping over forever to handle the
page fault on a single page.  However that should hardly happen, and after
all for each code path to return a VM_FAULT_RETRY we'll first wait for a
condition (during which time we should possibly yield the cpu) to happen
before VM_FAULT_RETRY is really returned.

This patch removes the restriction by keeping the FAULT_FLAG_ALLOW_RETRY
flag when we receive VM_FAULT_RETRY.  It means that the page fault handler
now can retry the page fault for multiple times if necessary without the
need to generate another page fault event.  Meanwhile we still keep the
FAULT_FLAG_TRIED flag so page fault handler can still identify whether a
page fault is the first attempt or not.

Then we'll have these combinations of fault flags (only considering
ALLOW_RETRY flag and TRIED flag):

  - ALLOW_RETRY and !TRIED:  this means the page fault allows to
                             retry, and this is the first try

  - ALLOW_RETRY and TRIED:   this means the page fault allows to
                             retry, and this is not the first try

  - !ALLOW_RETRY and !TRIED: this means the page fault does not allow
                             to retry at all

  - !ALLOW_RETRY and TRIED:  this is forbidden and should never be used

In existing code we have multiple places that has taken special care of
the first condition above by checking against (fault_flags &
FAULT_FLAG_ALLOW_RETRY).  This patch introduces a simple helper to detect
the first retry of a page fault by checking against both (fault_flags &
FAULT_FLAG_ALLOW_RETRY) and !(fault_flag & FAULT_FLAG_TRIED) because now
even the 2nd try will have the ALLOW_RETRY set, then use that helper in
all existing special paths.  One example is in __lock_page_or_retry(), now
we'll drop the mmap_sem only in the first attempt of page fault and we'll
keep it in follow up retries, so old locking behavior will be retained.

This will be a nice enhancement for current code [2] at the same time a
supporting material for the future userfaultfd-writeprotect work, since in
that work there will always be an explicit userfault writeprotect retry
for protected pages, and if that cannot resolve the page fault (e.g., when
userfaultfd-writeprotect is used in conjunction with swapped pages) then
we'll possibly need a 3rd retry of the page fault.  It might also benefit
other potential users who will have similar requirement like userfault
write-protection.

GUP code is not touched yet and will be covered in follow up patch.

dde1607248 mm: introduce FAULT_FLAG_DEFAULT
Although there're tons of arch-specific page fault handlers, most of them
are still sharing the same initial value of the page fault flags.  Say,
merely all of the page fault handlers would allow the fault to be retried,
and they also allow the fault to respond to SIGKILL.

Let's define a default value for the fault flags to replace those initial
page fault flags that were copied over.  With this, it'll be far easier to
introduce new fault flag that can be used by all the architectures instead
of touching all the archs.

With paging code made 5-level compliant, this is no longer needed.  ARC

code bloat due to needless inclusions of p*d_free_tlb() macros which
will be addressed in further patches.

2f4ecf68a0 ARC: mm: tlb flush optim: elide redundant uTLB invalidates for MMUv3
For MMUv3 (and prior) the flush_tlb_{range,mm,page} API use the MMU
TLBWrite cmd which already nukes the entire uTLB, so NO need for
additional IVUTLB cmd from utlb_invalidate() - hence this patch

local_flush_tlb_all() is special since it uses a weaker TLBWriteNI
cmd (prec commit) to shoot down JTLB, hence we retain the explicit
uTLB flush

1355ea2e60 ARC: mm: tlb flush optim: elide repeated uTLB invalidate in loop
The unconditional full TLB flush (on say ASID rollover) iterates over each
entry and uses TLBWrite to zero it out. TLBWrite by design also invalidates
the uTLBs thus we end up invalidating it as many times as numbe rof
entries (512 or 1k)

Optimize this by using a weaker TLBWriteNI cmd in loop, which doesn't
tinker with uTLBs and an explicit one time IVUTLB, outside the loop to
invalidate them all once.

And given the optimiztion, the IVUTLB is now needed on MMUv4 too where
the uTLBs and JTLBs are otherwise coherent given the TLBInsertEntry /
TLBDeleteEntry commands

cfd9d70a85 ARCv2: mm: TLB Miss optim: SMP builds can cache pgd pointer in mmu scratch reg
ARC700 exception (and intr handling) didn't have auto stack switching
thus had to rely on stashing a reg temporarily (to free it up) at a
known place in memory, allowing to code up the low level stack switching.
This however was not re-entrant in SMP which thus had to repurpose the
per-cpu MMU SCRATCH DATA register otherwise used to "cache" the task pdg
pointer (vs. reading it from mm struct)

The newer HS cores do have auto-stack switching and thus even SMP builds
can use the MMU SCRATCH reg as originally intended.

This patch fixes the restriction to ARC700 SMP builds only

8e3a68fb55 dma-mapping: make dma_atomic_pool_init self-contained
The memory allocated for the atomic pool needs to have the same
mapping attributes that we use for remapping, so use
pgprot_dmacoherent instead of open coding it.  Also deduct a
suitable zone to allocate the memory from based on the presence
of the DMA zones.

Pull force_sig() argument change from Eric Biederman:
 "A source of error over the years has been that force_sig has taken a
  task parameter when it is only safe to use force_sig with the current
  task.

  The force_sig function is built for delivering synchronous signals
  such as SIGSEGV where the userspace application caused a synchronous
  fault (such as a page fault) and the kernel responded with a signal.

  Because the name force_sig does not make this clear, and because the
  force_sig takes a task parameter the function force_sig has been
  abused for sending other kinds of signals over the years. Slowly those
  have been fixed when the oopses have been tracked down.

  This set of changes fixes the remaining abusers of force_sig and
  carefully rips out the task parameter from force_sig and friends
  making this kind of error almost impossible in the future"

23c0cbd0c7 ARCv2: entry: push out the Z flag unclobber from common EXCEPTION_PROLOGUE
Upon a taken interrupt/exception from User mode, HS hardware auto sets Z flag.
This helps shave a few instructions from EXCEPTION_PROLOGUE by eliding
re-reading ERSTATUS and some bit fiddling.

However TLB Miss Exception handler can clobber the CPU flags and still end
up in EXCEPTION_PROLOGUE in the slow path handling TLB handling case:

   EV_TLBMissD
     do_slow_path_pf
       EV_TLBProtV (aliased to call_do_page_fault)
          EXCEPTION_PROLOGUE

As a result, EXCEPTION_PROLOGUE need to "unclobber" the Z flag which this
patch changes. It is now pushed out to TLB Miss Exception handler.
The reasons beings:

 - The flag restoration is only needed for slowpath TLB Miss Exception
   handling, but currently being in EXCEPTION_PROLOGUE penalizes all
   exceptions such as ProtV and syscall Trap, where Z flag is already
   as expected.

 - Pushing unclobber out to where it was clobbered is much cleaner and
   also serves to document the fact.

 - Makes EXCEPTION_PROLGUE similar to INTERRUPT_PROLOGUE so easier to
   refactor the common parts which is what this series aims to do

926150db85 ARC: mm: do_page_fault refactor #8: release mmap_sem sooner
In case of successful page fault handling, this patch releases mmap_sem
before updating the perf stat event for major/minor faults. So even
though the contention reduction is NOT super high, it is still an
improvement.

There's an additional code size improvement as we only have 2 up_read()
calls now.

Note to myself:
--------------

1. Given the way it is done, we are forced to move @bad_area label earlier
   causing the various "goto bad_area" cases to hit perf stat code.

 - PERF_COUNT_SW_PAGE_FAULTS is NOW updated for access errors which is what
   arm/arm64 seem to be doing as well (with slightly different code)
 - PERF_COUNT_SW_PAGE_FAULTS_{MAJ,MIN} must NOT be updated for the
   error case which is guarded by now setting @fault initial value
   to VM_FAULT_ERROR which serves both cases when handle_mm_fault()
   returns error or is not called at all.

2. arm/arm64 use two homebrew fault flags VM_FAULT_BAD{MAP,MAPACCESS}
   which I was inclined to add too but seems not needed for ARC

 - given that we have everything is 1 function we can still use goto
 - we setup si_code at the right place (arm* do that in the end)
 - we init fault already to error value which guards entry into perf
   stats event update

Technically it seems like @bad_area label moved up, but even in old
regime, it was a special case of delivering SIGSEGV unconditionally
which we now do as well, although with checks.

Also note that @fault needs to be initialized since we can land in
@bad_area (which reads it) without setting it up with return value of
handle_mm_fault() - failing to do so did bite us although as a side
effect of different patch: see [1]

task (otherwise how are you synchronous) remove the task parameter
from from force_sig_fault to make it explicit that is what is going
on.

The two known exceptions that deliver a synchronous exception to a
stopped ptraced task have already been changed to
force_sig_fault_to_task.

The callers have been changed with the following emacs regular expression

a8c715b4dd ARC: mm: SIGSEGV userspace trying to access kernel virtual memory
As of today if userspace process tries to access a kernel virtual addres
(0x7000_0000 to 0x7ffff_ffff) such that a legit kernel mapping already
exists, that process hangs instead of being killed with SIGSEGV

Fix that by ensuring that do_page_fault() handles kenrel vaddr only if
in kernel mode.

And given this, we can also simplify the code a bit. Now a vmalloc fault
implies kernel mode so its failure (for some reason) can reuse the
@no_context label and we can remove @bad_area_nosemaphore.

Reproduce user test for original problem:

997aef68af init: provide a generic free_initmem implementation
Patch series "provide a generic free_initmem implementation", v2.

Many architectures implement free_initmem() in exactly the same or very
similar way: they wrap the call to free_initmem_default() with sometimes
different 'poison' parameter.

These patches switch those architectures to use a generic implementation
that does free_initmem_default(POISON_FREE_INITMEM).

This was inspired by Christoph's patches for free_initrd_mem [1] and I
shamelessly copied changelog entries from his patches :)

99bd5fcc50 ARC: PAE40: don't panic and instead turn off hw ioc
HSDK currently panics when built for HIGHMEM/ARC_HAS_PAE40 because ioc
is enabled with default which doesn't work for the 2 non contiguous
memory nodes. So get PAE working by disabling ioc instead.

Tested with !PAE40 by forcing @ioc_enable=0 and running the glibc
testsuite over ssh

do_page_fault() forgot to relinquish mmap_sem if a signal came while
handling handle_mm_fault() - due to say a ctl+c or oom etc.
This would later cause a deadlock by acquiring it twice.

This came to light when running libc testsuite tst-tls3-malloc test but
is likely also the cause for prior seen LTP failures. Using lockdep
clearly showed what the issue was.

a3010a0465 ARC: adjust memblock_reserve of kernel memory
In setup_arch_memory we reserve the memory area wherein the kernel
is located. Current implementation may reserve more memory than
it actually required in case of CONFIG_LINUX_LINK_BASE is not
equal to CONFIG_LINUX_RAM_BASE. This happens because we calculate
start of the reserved region relatively to the CONFIG_LINUX_RAM_BASE
and end of the region relatively to the CONFIG_LINUX_RAM_BASE.

For example in case of HSDK board we wasted 256MiB of physical memory:

518a2f1925 dma-mapping: zero memory returned from dma_alloc_*
If we want to map memory from the DMA allocator to userspace it must be
zeroed at allocation time to prevent stale data leaks.   We already do
this on most common architectures, but some architectures don't do this
yet, fix them up, either by passing GFP_ZERO when we use the normal page
allocator or doing a manual memset otherwise.

ARC, ARM, ARM64 and Unicore32 are all capable of parsing the "initrd="
command line parameter to allow specifying the physical address and size

3624379d90 ARC: IOC: panic if kernel was started with previously enabled IOC
If IOC was already enabled (due to bootloader) it technically needs to
be reconfigured with aperture base,size corresponding to Linux memory map
which will certainly be different than uboot's. But disabling and
reenabling IOC when DMA might be potentially active is tricky business.
To avoid random memory issues later, just panic here and ask user to
upgrade bootloader to one which doesn't enable IOC

This was actually seen as issue on some of the HSDK board with a version
of uboot which enabled IOC. There were random issues later with starting
of X or peripherals etc.

Also while I'm at it, replace hardcoded bits in ARC_REG_IO_COH_PARTIAL
and ARC_REG_IO_COH_ENABLE registers by definitions.

 "I have been slowly sorting out siginfo and this is the culmination of
  that work.

  The primary result is in several ways the signal infrastructure has
  been made less error prone. The code has been updated so that manually
  specifying SEND_SIG_FORCED is never necessary. The conversion to the
  new siginfo sending functions is now complete, which makes it
  difficult to send a signal without filling in the proper siginfo
  fields.

  At the tail end of the patchset comes the optimization of decreasing
  the size of struct siginfo in the kernel from 128 bytes to about 48
  bytes on 64bit. The fundamental observation that enables this is by
  definition none of the known ways to use struct siginfo uses the extra
  bytes.

  This comes at the cost of a small user space observable difference.
  For the rare case of siginfo being injected into the kernel only what
  can be copied into kernel_siginfo is delivered to the destination, the
  rest of the bytes are set to 0. For cases where the signal and the
  si_code are known this is safe, because we know those bytes are not
  used. For cases where the signal and si_code combination is unknown
  the bits that won't fit into struct kernel_siginfo are tested to
  verify they are zero, and the send fails if they are not.

  I made an extensive search through userspace code and I could not find
  anything that would break because of the above change. If it turns out
  I did break something it will take just the revert of a single change
  to restore kernel_siginfo to the same size as userspace siginfo.

  Testing did reveal dependencies on preferring the signo passed to
  sigqueueinfo over si->signo, so bit the bullet and added the
  complexity necessary to handle that case.

  Testing also revealed bad things can happen if a negative signal
  number is passed into the system calls. Something no sane application
  will do but something a malicious program or a fuzzer might do. So I
  have fixed the code that performs the bounds checks to ensure negative
  signal numbers are handled"

58b0440663 dma-mapping: consolidate the dma mmap implementations
The only functional differences (modulo a few missing fixes in the arch
code) is that architectures without coherent caches need a hook to
convert a virtual or dma address into a pfn, given that we don't have
the kernel linear mapping available for the otherwise easy virt_to_page
call.  As a side effect we can support mmap of the per-device coherent
area even on architectures not providing the callback, and we make
previous dangerous default methods dma_common_mmap actually save for
non-coherent architectures by rejecting it without the right helper.

In addition to that we need a hook so that some architectures can
override the protection bits when mmaping a dma coherent allocations.

dd45210b6d ARC: don't check for HIGHMEM pages in arch_dma_alloc
__GFP_HIGHMEM flag is cleared by upper layer functions
(in include/linux/dma-mapping.h) so we'll never get a
__GFP_HIGHMEM flag in arch_dma_alloc gfp argument.
That's why alloc_pages will never return highmem page
here.

Get rid of highmem pages handling and cleanup arch_dma_alloc
and arch_dma_free functions.

2820a708d5 ARC: dma [IOC] Enable per device io coherency
So far the IOC treatment was global on ARC, being turned on (or off)
for all devices in the system. With this patch, this can now be done
per device using the "dma-coherent" DT property; IOW with this patch
we can use both HW-coherent and regular DMA peripherals simultaneously.

The changes involved are too many so enlisting the summary below:

1. common code calls ARC arch_setup_dma_ops() per device.

2. For coherent dma (IOC) it plugs in generic @dma_direct_ops which
   doesn't need any arch specific backend: No need for any explicit
   cache flushes or MMU mappings to provide for uncached access

   - dma_(map|sync)_single* return early as corresponding dma ops callbacks
     are NULL in generic code.
     So arch_sync_dma_*() -> dma_cache_*() need not handle the coherent
     dma case, hence drop ARC __dma_cache_*_ioc() which were no-op anyways

3. For noncoherent dma (non IOC) generic @dma_noncoherent_ops is used
   which in turns calls ARC specific routines

   - arch_dma_alloc() no longer checks for @ioc_enable since this is
     called only for !IOC case.

50a7ca3c6f mm: convert return type of handle_mm_fault() caller to vm_fault_t
Use new return type vm_fault_t for fault handler.  For now, this is just
documenting that the function returns a VM_FAULT value rather than an
errno.  Once all instances are converted, vm_fault_t will become a
distinct type.

Ref-> commit 1c8f422059 ("mm: change return type to vm_fault_t")

In this patch all the caller of handle_mm_fault() are changed to return
vm_fault_t type.

4c612add7b ARC: dma [non IOC]: fix arc_dma_sync_single_for_(device|cpu)
ARC backend for dma_sync_single_for_(device|cpu) was broken as it was
not honoring the @dir argument and simply forcing it based on the call:
 - arc_dma_sync_single_for_device(dir) assumed DMA_TO_DEVICE (cache wback)
 - arc_dma_sync_single_for_cpu(dir) assumed DMA_FROM_DEVICE (cache inv)

This is not true given the DMA API programming model and has been
discussed here [1] in some detail.

Interestingly while the deficiency has been there forever, it only started
showing up after 4.17 dma common ops rework, commit a8eb92d02d
("arc: fix arc_dma_{map,unmap}_page") which wired up these calls under the
more commonly used dma_map_page API triggering the issue.

address translation or mapping [1].  Ensure they actually do that by calling

The now unused _dma_cache_sync function is removed as well.

[1] in fact various drivers rely on that by passing DMA_ATTR_SKIP_CPU_SYNC
to the map/unmap routines and doing the cache synchronization manually.

3eb0f5193b signal: Ensure every siginfo we send has all bits initialized
Call clear_siginfo to ensure every stack allocated siginfo is properly
initialized before being passed to the signal sending functions.

Note: It is not safe to depend on C initializers to initialize struct
siginfo on the stack because C is allowed to skip holes when
initializing a structure.

The initialization of struct siginfo in tracehook_report_syscall_exit
was moved from the helper user_single_step_siginfo into
tracehook_report_syscall_exit itself, to make it clear that the local
variable siginfo gets fully initialized.

In a few cases the scope of struct siginfo has been reduced to make it
clear that siginfo siginfo is not used on other paths in the function
in which it is declared.

Instances of using memset to initialize siginfo have been replaced
with calls clear_siginfo for clarity.

cb9f753a37 mm: fix races between swapoff and flush dcache
Thanks to commit 4b3ef9daa4 ("mm/swap: split swap cache into 64MB
trunks"), after swapoff the address_space associated with the swap
device will be freed.  So page_mapping() users which may touch the
address_space need some kind of mechanism to prevent the address_space
from being freed during accessing.

The dcache flushing functions (flush_dcache_page(), etc) in architecture
specific code may access the address_space of swap device for anonymous
pages in swap cache via page_mapping() function.  But in some cases
there are no mechanisms to prevent the swap device from being swapoff,
for example,

    flush_dcache_page()
      mapping = page_mapping()
        ...				  exit_swap_address_space()
        ...				    kvfree(spaces)
        mapping_mapped(mapping)

The address space may be accessed after being freed.

But from cachetlb.txt and Russell King, flush_dcache_page() only care
about file cache pages, for anonymous pages, flush_anon_page() should be
used.  The implementation of flush_dcache_page() in all architectures
follows this too.  They will check whether page_mapping() is NULL and
whether mapping_mapped() is true to determine whether to flush the
dcache immediately.  And they will use interval tree (mapping->i_mmap)
to find all user space mappings.  While mapping_mapped() and
mapping->i_mmap isn't used by anonymous pages in swap cache at all.

So, to fix the race between swapoff and flush dcache, __page_mapping()
is add to return the address_space for file cache pages and NULL
otherwise.  All page_mapping() invoking in flush dcache functions are
replaced with page_mapping_file().

92d4412824 ARCv2: Accomodate HS48 MMUv5 by relaxing MMU ver checking
HS48 cpus will have a new MMUv5, although Linux is currently not
explicitly supporting the newer features (so remains at V4).
The existing software/hardware version check is very tight and causes
boot abort. Given that the MMUv5 hardware is backwards compatible,
relax the boot check to allow current kernel support level to work
with new hardware.

Also while at it, move the ancient MMU related code to under ARCompact
builds as baseline MMU for HS cpus is v4.

1ee55a8f7f ARC: Re-enable MMU upon Machine Check exception
I recently came upon a scenario where I would get a double fault
machine check exception tiriggered by a kernel module.
However the ensuing crash stacktrace (ksym lookup) was not working
correctly.

Turns out that machine check auto-disables MMU while modules are allocated
in kernel vaddr spapce.

This patch re-enables the MMU before start printing the stacktrace
making stacktracing of modules work upon a fatal exception.

[Needed for HSDK]

Currently the first page of system (hence RAM base) is assumed to be
@ CONFIG_LINUX_LINK_BASE, where kernel itself is linked.

However is case of HSDK platform, for reasons explained in that patch,
this is not true. kernel needs to be linked @ 0x9000_0000 while DDR
is still wired at 0x8000_0000. To properly account for this 256M of RAM,
we need to introduce a new option and base page frame accountiing off of
it.

This patch introduces consistency allowing platforms to switch to less
verbose console logging using cmdline.
